安多泰(Endotine)在中下面部除皱术中的应用 被引量:4

Application of Endotine technique in mid-lower facial rhytidectomy

摘要 目的为了提高除皱术的效果,延长效果持续时间,从理论与实践中尝试在小切口除皱的基础上,应用安多泰抓提面部深部软组织,使其解剖复位,以达到恢复面部年轻时的轮廓,从而达到年轻化的目的。方法颧部抓提颧脂肪垫,向后上方以45°角适度力量牵引固定于颞深筋膜上;颊部抓提SMAS,垂直方向以适度力量向上提拉,固定于颞深筋膜上;颈部抓提颈阔肌近中线区域,向后上方以适度力量牵引,固定于耳后乳突骨膜上。结果本组共47例患者,62个部位,随访3~12个月,其中1例单侧悬吊带断裂,4例早期有不同程度的面瘫表现,余患者均对除皱效果满意。结论安多泰是一种由可降解的生物材料制成的不同形状及功能的除皱辅助器械材料,可使面部的软组织解剖复位,增加局部软组织容量,同时也克服了传统除皱术中间接提紧软组织,作用区域提升不足的弊病,是一种值得尝试的临床技术。 Objective To introduce the clinical experience on the application of Endotine to small incision rhytidectomy to enhance the effect and extend the duration of rhytidectomy and to explore the efficacy of Endotine in grasping and pulling upward the deep facial soft tissue to anatomical reconversion to restore a youthful facial contour in order to achieve the rejuvenation purpose. Methods Zygomatic malar fat pad was grasping and pulling upward 45° angle by moderate strength and fixed on the deep temporal fascia; malar SMAS was grasping and pulling vertically upwards by modest force and fixed on the deep temporal fascia; platysma nearby midline region in the neck was grasping and pulling upward and back by moderate strength and fixed on the post anr mastoid periosteum. Results A total of 47 patients (62 operate sites) were received in this study. After 3 - 12 months followup, all cases showed satisfactory results except 1 case with the complication of unilateral suspension fracture and 4 cases with facial paralysis performance postoperatively. Conclusion Endotine is a biodegradable material of different shapes and functions for rhytidectomy. We can make facial soft tissue anatomical reconversion, increase the capacity of local soft tissue by Endotine and overcome the shortcomings of insufficient to enhance the target region by indirect pulling upward the soft tissues in traditional rhytidectomy. It is worth spreading in clinic.
