丙泊酚对进行机械通气病人的镇静治疗效果

Clinical Observation of Propofol Sedation Effect on Patient with Mechanical Ventilation in Respiratory ICU

摘要 目的探讨丙泊酚在机械通气病人中的镇静效果。方法将89例进行机械通气的患者分为丙泊酚组(46例)和咪唑安定组(43例),观测2组患者镇静等级达Ramsay分级3~5级的时间,给药前后呼吸、循环参数的变化,停药后患者清醒时间等指标并进行比较。结果丙泊酚镇静治疗起效快,达到满意镇静深度时间〔(23.46±2.7)min〕优于咪唑安定组〔(31.56±5.3)min〕(t=8.99,P<0.01)用药后血压、心率逐渐稳定,呼吸频率、血氧饱和度、动脉氧分压等亦有明显改善(P<0.05)。丙泊酚停药后清醒时间短〔(22.50±3.8)min〕,与咪唑安定组〔(115.28±5.5)min〕差异有统计学意义(t=91.41,P<0.01)。结论丙泊酚是机械通气患者较理想的的镇静药物。 [Objective]To study the sedation effect of Propofol on patients with mechanical ventilation.[Methods]For propofol group and midazolam group,the achieving Ramsay Grade 3~5 sedation effect time and the change of breath and circulatory before and after taking medicine,as well as the clear time of sober up after medicine stopped were observed and compared.[Results]The effect of propofol sedative therapy onset rapidly,achieving satisfactory sedation time 〔(23.46±2.7)min〕was better that of midazolam group〔(31.56±5.3)min〕(t=8.99,P0.01). The blood pressure and heart rate was stabilized gradually after treatment. The respiratory frequency,the saturation of oxygen in blood and partial pressure of arterial oxygen also improved significantly(P0.05). Awaking time was short after stopping propofol 〔(22.50±3.8)min〕,which was significantly different with midazolam group〔(115.28±5.5)min〕 (t=91.41,P0.01).[Conclusion]Popofol is an ideal sedative medicine for patient with mechanical ventilation.
