非球面切削LASIK治疗-10D以上高度近视 被引量:15

Aspherical keratectomy excimer laser in situ keratomileusis(LASIK)for correction of myopia higher than -10D

摘要 为评估非球面切削准分子激光原位角膜磨镶术(LASIK)治疗-10D以上高度近视的有效性及安全性。采用SCMD气动式角膜切割刀及SCHWINDKERATOM—F型准分子激光系统,利用Ultravision非球面切削程序对-10D以上高度近视(等值球镜-10.25~-30.00D)117例180只眼行LASIK手术,其中屈光度-10.25D~-15.00D(A组,超高度近视)78只眼,平均-12.62±1.52D;屈光度-15.50D~-30.00D(B组,极高度近视)102只眼,平均-19.18±3.38D。术后随访观察6个月以上。结果:术后6个月裸眼视力≥1.0者A、B二组分别达57.7%及25.5%,裸眼视力≥0.5者分别达94.9%及76.5%;屈光度在±1.00D以内的分别达41.0%及19.6%,在±3.00D以内的分别达83.3%及51.0%。A、B二组各有1眼(分别占1.3%及1.0%)术后3个月有黄斑出血,6个月时的最佳矫正视力与术前相比,低于2行以上。结论:非球面切削LASIK手术对于矫治-10D以上高度近视有很好的疗效,影响手术安全性的主要因素为视网膜并发症。 Objective:To study the efficacy and safety of aspherical keratectomy excimer laser in situ keratomileusis for correction of myopia higher than -10D.Methods:We studied 180 eyes of 117 patients with high myopia of spherical equivalent of -10 25D to -30 00D who received aspherical keratectomy LASIK with SCMD microkeratome and SCHWIND KERATOM F excimer laser system.The eyes were divided into two groups,group A(spherical equivalent of -10 25D to -15 00D,average -12 62±1 52D)78 eyes and group B(spherical equivalent of -15 50D to -30 00D,average -19 18±3 38D)102 eyes.All patients have been followed up for 6 months or longer.Results: Six months after surgery,uncorrected visual acuity was 1 0 or better in 45 eyes of group A (47 6%) and 26 eyes of group B(25 5%).0 5 or better in 74 eyes of group A (94 9%)and 78 eyes of group B(76 5%).In group A and B,32 eyes (41 0%) and 20 eyes (19 6%) had a refraction within ±1 00D;65 eyes (83 3%) and 52 eyes (51 0%) had a refraction within ±3 00D.In group A and B,loss of 2 or more lines of best corrected visual acuity was observed in 1 3% and 1 0% eyes with maculae heamorrhage.Couclusion:Aspherical keratectomy LASIK is an effective and good method to correct myopia higher than -10D,with minimal complications.The factors affecting the safety of surgery are retinal complications.
