摘要
目的：为探讨咽旁隙肿瘤的外科治疗方法，进一步减少术后并发症，提高手术成功率。方法：本文回顾性总结１９８１～１９９４年以来４５例咽旁隙肿瘤的外科治疗，其中良性肿瘤４０例，恶性肿瘤５例，根据患者的实际情况，采用了４种手术入路。结果：肿瘤全切４４例，术后颅神经并发症１１例（神经源性肿瘤８例，腮腺肿瘤３例），其中８例于术后６个月之内恢复，随诊中，良性肿瘤术后复发３例，恶性肿瘤术后３年生存率４０％（２／５）。结论：结果表明，咽旁隙肿瘤术后并发症主要为颅神经功能障碍。仔细解剖其毗邻关系和剥离肿瘤是手术成功的关键，大多数患者可望恢复，并对咽旁隙手术的进路，肿瘤的病理类型，手术要点及术后并发症进行总结和讨论。
Objective:To explore methods of surgical treatment of parapharyngeal space tumors,decrease postoperative complications and promote success rate of the operation.Methods:Surgical treatment in 45 cases with parapharyngeal space tumors between 1981 and 1994 was summarized retrospectively.Of the 45 cases, 40 were benign tumors and 5 were malignant neoplasms.According to the various locations of tumor,4 operative approaches were applied to the resection of tumors.Results:Of the 45 cases,tumors were totally resected in 44 cases,11 cases were discovered to have developed complications of cranial nerves and 8 of the 11 cases recovered to normal within 6 months,postoperatively; in 3 of the 40 benign cases tumors recurred and the 3 year survival rate of malignant neoplasms was 40%(2/5)during the period of 3 year follow up.Conclusion:Postoperative complications of the parapharyngeal space tumors were mainly dysfunction of the cranial nerves.As every main part in the space is dissected and the tumor is separated very carefully,most of the patients suffering from cranial nerve complications may hopefully return to normal.The anatomy of parapharyngeal space,pathological types of the tumors,operative key points and postoperative complications have been summarized and discussed.
