摘要
目的通过分析2005-2007年湖南省各市(州)网络直报疑似肺结核病人报告现状,评价结核病防治机构与非结核病防治机构的合作和协调情况。方法对近3年结核病信息管理系统湖南省各市(州)非结核病防治机构网络直报疑似肺结核病人的肺结核病例报告卡,剔除重报病例和已到结核病防治机构就诊的病例,获取未到结核病防治机构就诊病人的相关信息,分析其转诊到位率、追踪率及总体到位率变化及其原因。结果2005-2007年湖南省非结防机构网络直报肺结核病人数分别是26 521例,38 840例和57 305例,排除重报和辖区外患者数,则应转诊人数分别是20 623例,33 288例和47 649例。2005-2007年转诊到位率分别是45.23%,39.02%,28.83%;追踪到位率分别是49.76%,47.02%,53.82%;总体到位率分别是68.19%,65.90%,64.97%。结论湖南省各市(州)非结核病防治机构网络直报疑似肺结核病人转诊率、追踪率没有达到国家的要求。需加强综合医院医生培训与宣传,提高结防机构和非结防机构的合作,以提高结核病的发现水平。
Objective To evaluate the situation of cooperation and coordination between tuberculosis（TB） control agencies and the other related institutes by analyzing the status of TB suspected patients,who were directly reported through network in cities of Hunan province.Methods The outcomes of direct network report cards were analyzed for the year of 2005 to 2007 in cities of Hunan province by removing redundant TB cases.The outcomes included patients receiving treatments in TB control agencies,patients following up for the disease condition or no further records of treatment or disease development.Results The direct network report had 26 521,38 840 and 57 305 TB suspected cases from year of 2005 to 2007 respectively in non-tuberculosis agencies among cities of Hunan province.The expected cases were 20 623,33 288 and 47 649 from year of 2005 to 2007 by removing redundancies.The rate of referral in place,of following-up information and of any additional records available for the TB suspected patients were 45.23%,49.76%,68.19% for the year of 2005;and were 39.02%,47.02%,65.90% for the year of 2006;and were 28.83%,53.82%,64.97% for the year of 2007,respectively.Conclusion The rate of referral and following-up for TB suspected cases reported by network did not meet national requirement.It is important to provide additional TB training for doctors,have more advocacy in general hospitals and improve communication between TB control agencies and non-TB control institutes.
