硬膜外联合全身麻醉对老年腹部手术患者内脏氧合的影响 被引量:4

摘要 目的:观察硬膜外联合全身麻醉对老年腹部手术患者内脏氧合的影响。方法:30例老年腹部手术患者随机分为全身麻醉组(G组,15例)和硬膜外联合全身麻醉组(G+E组,15例)。两组患者麻醉前经鼻放置胃张力计导管,麻醉诱导、维持均按相同公斤体重给药。G+E组先行硬膜外置管,手术开始前10min硬膜外腔泵注罗哌卡因。常规监测循环、呼吸功能、血气、胃黏膜二氧化碳分压(PgCO2)。分别于麻醉诱导前(T0)、气管插管后2min(T1)、手术持续60min(T2)、手术持续120min(T3)、手术持续180min(T4)测定并记录各监测指标。结果:两组患者T1时心率差异具有显著性(P<0.05),G+E组T1时平均动脉压波动差异组内有显著性(P<0.05);G组的血乳酸及碱剩余(BE)在T4处组间、组内差异有显著性(P<0.05);G组的pHi变化分别在T3、T4处组间、组内差异有显著性(P<0.05)。结论:硬膜外联合全身麻醉可降低气管插管反应;对于长时间的麻醉,硬膜外联合全身麻醉可以减轻内脏氧合严重失当问题,相对于血乳酸、碱剩余,pHi是监测内脏氧合问题的敏感、特异性指标。
