摘要
目的评估带血管蒂头状骨移位替代术治疗晚期kienbock病的手术疗效。方法A组:13例晚期(Licht-manⅢ、Ⅳ期)kienbock病患者,入院后均行腕部X线检查、头状骨移位替代术。B组:11例行月骨假体植入术。结果以术后12个月随访结果为准。A组除差1例,12例腕痛基本消失、平均握力恢复至健侧的76%,腕关节活动范围为健侧的51%。结论头状骨移位替代术治疗晚期kienbock病,能缓解疼痛、改善握力及腕关节活动度。
Objective To evaluate clinical therapeutic effect of Capitate transposition for treatment of kienbock＇s disease in advanced stage （stageⅢ,Ⅳ）.Methods 13 cases of kienbock＇s disease（Grounp A） in advanced stage were involved in the study.The cases were excised with capitate transposition.X-ray films of the wrist were taken in 13 individuals.11cases （Grounp B） were excised with lunate prosthesis implant.Results All patients were followed.The follow-up result of the sixth month is the evaluated standard of postoperation.With the exception of 1 cases,no pain or almost painless result was obtained in 12 patients （Grounp A）.The grip strength and range of motion averaged 76.92% and 45.45% of opposite wrist.Conclusion Capitate transposition is an effective methods to treat kienbock＇s disease in advanced stage,which could make a painless wrist with useful grip strength and preservation of wrist motion。
