摘要
目的观察血液净化联合中药治疗重型肝炎合并肾功能衰竭对降低其病死率的作用。方法对２７例重型肝炎合并肾功能衰竭患者采用血液净化疗法联合中药治疗。结果２７例中８例存活，１９例死亡，４例直接死于肾功能衰竭。结论该疗法在降低重型肝炎合并肾功能衰竭病死率可能有一定效果，但应早期诊断、早期治疗。
Objective To evaluate a combination treatment with hemopurification and Chinese herbs in reducing mortality of severe viral hepatitis complicated acute renal failure. Methods Subjects in the present study consisted of 27 patients (21 male, 6 female, mean age 41 5 years) with severe viral hepatitis complicated acute renal failure, 12 with simultaneously presented cerebral edema. The Patients received a combined treatment with hemopurification and Chinese herbs from 1990 to 1996 Three types (PE, CAVHD, HD) of hemopurification were selected, and Chinese herbs were used by oral administration or enema before and after hemopurification. Results After treatment, 8 of 27 patients survived, 7 of 15 patients with severe viral hepatitis and acute renal failure survived. Conclusions Hemopurification, PE+CAVHD OR HD can clear poisonous substances in serum, decrease the serum BUN, CR, improve renal function, reduce cerebral edema, and adjust electrolyte and osmotic pressure. Chinese herbs can help improve micro-circulation before and after hemopurification. The results suggest it is necessary to diagnose and treat severe viral hepatitis early, and the combination treatment is worth attempting in reducing mortality of severe viral hepatitis complication acute renal failure.
