摘要
叶天士《临证指南医案.咳嗽》中汇集了有关诊治咳嗽的医案142例,归纳为风寒袭肺、风热犯肺、肺燥津伤、肝火灼肺、脾虚肺弱和肺肾虚衰6型。通过分析,可以看出叶天士治疗咳嗽,并非见咳治咳,而是善于掌握病因病机。邪在表者予轻剂宣解,邪在里者或不治肺而治脾肾,注重顾护胃气和津液。充分体现了叶天士辨治咳嗽精深的学术见解与博通丰富的临证经验。
Clinical Guide to Medical Records is a collection of YE Tian-shi＇s succeeding cases and prescriptions on various diseases of internal medicine, gynecological and pediatric diseases. It contained a total of 142 cases related to diagnosis and treatment of cough, which were roughly divided into six types, wind-cold pulmonary sthenia, wind-heat pulmonary sthenia, consumption of body fluid due to lung-dryness, liver heat invading lung, hypofunction of spleen leading to weakness of lung, and deficiency of lung and kidney. By the prescriptions in every case, we can see that YE Tian-shi did not like to kill the symptom of cough, but grasped the cause and pathogenesis of cough to kill cough while he treated it. When evil was in the exterior, he would select the prescription for expelling wind-evil to open the inhibited lung-energy. But when evil was in the interior, he would not treat lung but treat spleen and kidney, focusing on protecting gastric qi and body fluid. All those fully reflect YE Tian-shi＇s treatment characteristics, meticulous differentiation of symptoms and signs and good at seizing the principal syndrome.
