摘要
【目的】观察中药胃疡生肌汤对难治性消化性溃疡疗效的影响。【方法】将60例难治性消化性溃疡患者随机分为2组,每组30例。对照组给予西药三联疗法(灭滴灵、阿莫西林、洛赛克)治疗,治疗组在对照组三联疗法的基础上加用中药胃疡生肌汤(由党参、白术、茯苓、白芍、白花蛇舌草、黄芪、天花粉、郁金、佛手、甘草、三七粉、白及粉等组成)治疗,疗程均为2个月。主要观察临床疗效、幽门螺旋杆菌(HP)根除情况及治疗前后血浆内皮素-1(ET-1)水平等指标的变化。【结果】(1)临床疗效方面,治疗组总有效率为90.0%,对照组为76.7%,两组比较,治疗组疗效优于对照组(P<0.05)。(2)治疗后HP根除率治疗组为92.9%,对照组为92.6%,两组比较差异无显著性意义(P>0.05)。(3)治疗后两组血浆ET-1水平均显著升高(与治疗前比较,P<0.05或P<0.01),且治疗组的升高作用优于对照组(P<0.05)。【结论】中药胃疡生肌汤能显著提高西药三联疗法治疗难治性消化性溃疡的疗效,这可能与其促进ET-1的生成作用有关。
Objective To observe the effect of Weiyang Shengji Decoction （WSD） on refractory peptic ulcer （RPU）. Methods Sixty RPU patients were equally randomized into 2 groups. The control group received triple therapy of metronidazole, amoxicillin and omeptazole. The treatment group received WSD （ mainly composed of Radix Angelicae Sinensis, Rhizoma Atractylodis Macrocephalae, Poria, Radix Paeoniae Alba, Herba Hedyotis Diffusae, Radix Astragali, powder of Radix Trichosanthis, Radix Curcumae, Fructus Citri Sarcodactylis, Radix Glycyrrhizae, powder of Radix Notoginseng, powder of Rhizoma Bletillae ） besides the triple therapy. After treatment for 2 months, the clinical symptoms, gastroscopic results and helicobacter pylori （HP） eradication rate were observed, and the plasma endothelin 1 （ ET-1 ） level was detected. Results After treatment for 2 months, the total effective rate was 90. 0% in the treatment group and 76.7% in the control group （ P 〈 0. 05 ） ; HP eradication rate was 92. 9% in the treatment group and 92. 6% in the control group （P 〉0.05） ; the plasma ET-1 level was increased in the two groups （ P 〈 0.05 or P 〈 0.01 compared with that before treatment）, and the increase was obvious in the treatment group （P 〈 0. 05 ）. Conclusion Herbal medicine of WSD can enhance the effect of western medicine for RPU, and its mechanism may be related with the promotion of ET-1 formation.
