胫骨平台内翻角的测量分析与临床疗效研究 被引量:9

Measurement of varus inclination of the proximal tibial and its relevance of functional recovery

摘要 [目的]探讨复杂胫骨平台骨折手术前后胫骨平台内翻角的改变以及与膝关节功能恢复的相关性。[方法]于2005年1月~2008年12月收治复杂胫骨平台骨折病例36例,分别用PACS图像分析软件测量术前及术后胫骨平台内翻角的度数,术后进行膝关节功能评分,用SPSS 14.0统计软件进行统计分析。[结果]复杂骨折手术治疗后胫骨平台内翻角为4.18°,与中国陶坤、吴海山教授所报道的正常胫骨平台内翻角数值进行统计学比较,发现无明显统计学差异(P>0.05),用Spearman相关分析发现手术后胫骨平台内翻角纠正程度与膝关节功能之间存在相关性(r=0.812,P<0.01)。[结论]复杂胫骨平台骨折手术前后胫骨平台内翻角发生明显的变化,术后胫骨平台内翻角的纠正程度对膝关节功能的恢复有一定的影响。 [Objective]To measure the preoperative and postoperative varus inclination of the proximal tibial in the complex titial plateau fracture,and to explore the relevance between the angle changes and the knee functional recovery.[Method]From January 2005 to December 2008,36 cases of complex tibial plateau fracture were treated.Their titial plateau preoperative and postoperative varus angles were measured by PACS images analysis software,knee joint stability was measured by Rasmussen's functional grading system and SPSS14.0 was used for statistical analysis.[Result]Postoperative tibial plateau varus angle was 4.18o,showing no significant difference from Chinese normal tibial plateau varus angle reported by TAO Kun,WU Hai-shan,et al(P〉0.05).Spearman analysis revealed correlation between postoperative3 tibial plateau varus correction angle and functional recovery of knee joint(r=0.812,P〈0.01).[Conclusion]The tibial plateau varus angle has significant changes after surgical treatment,there is a certain relation between the correction of tibial plateau angle and recovery of knee joint function.
