摘要
目的:探讨经枕下-乙状窦后入路显微手术切除听神经瘤保留面神经功能的可能性和可行性。方法:对42例听神经瘤患者均采取经枕下-乙状窦后入路显微手术行肿瘤切除术。结果:肿瘤全切除34例,次全切除8例。面神经解剖保留33例,出院时面神经功能保留率为52.4%(HouseⅠ～Ⅱ级22例)。平均随访5年,其中29例恢复良好,10例恢复一般,3例恢复较差。结论:经枕下-乙状窦后入路显微外科手术治疗听神经瘤是安全有效的,有利于面神经功能的保留。
Objective： To investigate the feasibility of removing the acoustic neuromas via suboccipito-retrosigmoid approach by microsurgery. Methods： The acoustic neuromas in 42 patients were removed by microsurgy via suboccipito-retrosigmoid approach. Results：The tumors were totally removed in 34 cases and partly removed in 8 cases. Anatomic preservation of the facial nerve was achieved in 33 cases and functional preservation in 22 cases （ House Grade Ⅰ-Ⅱ ） at discharge. All the cases were followed up for an average of 5 years. The result was good in 29 cases, fair in 10 and poor in 3. Conclusions：h is safe and effective to remove the acoustic neurcoma via suboccipito-retrosigmoid approach using microsurgical technique.
