CA系列凝血分析仪凝血三项测定结果的可比性研究 被引量:14

摘要 [目的]探讨同一系列不同型号凝血分析仪检测凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)和纤维蛋白原(Fg)的结果是否具有可比性。[方法]收集40例不同浓度的患者新鲜血浆,在两个不同的凝血分析仪(CA1500、CA7000)测定PT、APTT和Fg,并对结果进行统计学分析,以医学决定水平处的系统误差来判断检测系统间的结果是否为临床接受。[结果]CA7000的PT测定结果与CA1500比较,差异无显著性意义(P>0.05),相关系数(r)=0.999;CA7000的INR测定结果与CA1500比较,差异无显著性意义(P>0.05),r=0.999;CA7000的APTT测定结果与CA1500比较,差异无显著性意义(P>0.05),r=0.998;CA7000的Fg测定结果与CA1500比较,差异有显著性意义(P<0.01),r=0.985。医学决定水平处的系统误差大部分小于总允许误差的1/2(PT:T±7.5%;APTT:T±7.5%;Fg:T±10%)。[结论]以CA1500作为标准检测系统,CA7000的测定结果具有临床可接受性;应定期对同一实验室不同凝血分析仪进行比对实验,以保证检验结果间的可比性。 [ Objective ] To discuss the comparability of PT, AFFF and Fg results between different coagulation analyzers of the same series. [ Methods ] We collected 40 patients' plasma and detected PT, APTT and Fg by two coagulation analyzers (CA1500, CA7000) respectively. The results were statistically analyzed and clinical acceptability was evaluated by the systemic error at medical determining level. [ Results ] There was no significant difference between PT results detected by CA7000 and those detected by CA1500 (P 〉 0.05) and the correlation coefficient was 0. 999. There was no significant difference between INR results detected by CA7000 and those detected by CA1500 (P 〉 0.05 ) and the correlation coefficient was 0. 999. There was no significant difference between APTr results detected by CAT000 and those detected by CA1500 (P 〉 0.05 ) and the correlation coefficient was 0.998. There was significant difference between Fg results detected by CA7000 and those detected by CA1500 (P 〈0.01 ) and the correlation coefficient was 0.985. Most of the systemic errors were less than half of the limited systemic error ( PT: T ± 7.5 % ; APTT: T ±7.5 % ; Fg: T ± 10% ). [ Conclusion ] The results of CA7000 are acceptable compared to the standard analyzer CA1500. Comparison study must be done periodically among different coagulation analyzers in the same laboratory in order to ensure the comparability of different results.
