摘要
目的观察头孢曲松钠、七叶皂苷和巴曲酶联合治疗急性进展型脑梗死的疗效．方法将120例急性进展型脑梗死患者随机分入联合治疗组和巴曲酶组，每组各60例。联合治疗组予注射用头孢曲松钠2g，注射用七叶皂苷10mg，分别静咏滴注，每日一次，连用14d，同时在第1、3、5、7天静脉滴注巴曲酶，剂量分别是10、5、5、5BU。巴曲酶组则单用巴曲酶治疗，用法同联合治疗组。两组治疗前后均进行中国卒中程度评分及凝血指标检查。结果联合治疗组总有效率（97％）明显高于巴曲酶组（65％）（P〈0．05）。治疗后两纽的血浆纤维蛋白原水平均明显降低（均P〈0．05），血小板计数、活化的部分凝血活酶时间、凝血酶原时间、凝血酶时间差异均无统计学意义。结论头孢曲松钠和七叶皂苷联合巴曲酶治疗急性进展型脑梗死疗效显著，无明显不良反应。
Objective To evaluate the curative effects of cefiriaxone sodium and aescine injection（ganglioside）combined with batroxobin in treatment of acute progressive cerebral infarcion （APCI）. Methods Eighty APCI patients were divided randomly and equally into two groups with 60 in each group. Combined treatment group was treated with ceftriaxone 2g and aescine 10 mg iv,gtt,qd for 14 days;as well as batroxobin iv, gtt,on 1 st（ 10 BU） ,3rd（ 5 BU ） ,5th day（ 5 BU ） ,7th day （5 BU ）. Control group was treated with batroxobin only, the same splitting dosages as those in combined treatment group. Chinese Stroke Scale（CSS） ,modified Rankin Scale（mRS） and blood clotting index were taken before and after the treatment in both groups. Results There was significant difference of clinical efficacy between the two groups（P 〈 0.05 ）. The total effective rate of combined treatment group（96% ）was obviously higher than that of control group （65%） （P 〈 0.05 ）. The plasma fibrinogen levels in both groups decreased significantly after the treatment（P 〈 0.05 ）. No significant difference was found in platelets counting, activated partial thrombinplastin time,prothrombin time and thrombin lime before and after the treatment in both groups. Conclusion Ceftriaxone and aeseine combined with batroxohin in remarkably effective in the treatment of APCI without obvious adverse reactions.
