摘要
目的:评价MRI脑线性测量方法对早期Alzheimer's病(AD)的诊断价值。方法:分别对22例轻度痴呆的AD患者和20例正常老年人进行纵裂宽度,大脑横径,侧脑室额角宽度,双额指数,中颞叶最小厚度,钩回间距,海马高度,脉络膜裂宽度和颞角宽度等指标测量。结果:显示颞角增宽是鉴别AD与正常老年人最敏感的指标,结合应用脉络膜裂宽度,中颞叶最小厚度,钩回间距和大脑横径等指标诊断AD的敏感度为86.4%,特异度为85.0%,总的诊断准确率为85.7%。
Purpose:To evaluate the diagnostic value of MR image linear measurement of brain in Alzheimer's disease (AD) in the early stage. Methods:The interhemispheric fissure width,cerebral transverse diameter,width of anterior horn of lateral ventricle,bifrontal index,minimal thickness of medial temporal lobe,interuncal distance,hippocampal height,width of the choroid fissure and width of the temporal horn of lateral ventricle were measured by MRI in 22 patients with mild AD and 20 normal senile subjects subjects respectively.Results:The most sensitive index was increasing the width of the temporal horn of lateral ventricle in discriminating AD patients from normal senile subjects.The index combined with increasing width of the choroid fissure and interuncal distance,as well as decreasing cerebral transverse diameter,the sensitivity,specificity and accuracy of diagnostic AD respectively were 864%,850% and 857%.Conclusion:The linear measurement of hippocampal and temporal atrophy is a one of method in diagnosis of mild AD.
