摘要
目的分析和总结取卵日取精困难患者的精子用于卵泡浆内单精子显微注射-胚胎移植的实验室和临床结局。方法收集11对取卵日获取精液失败后行经皮附睾穿刺抽吸术和(或)睾丸细针抽吸术并进行卵胞浆内单精子显微注射的夫妇助孕结局。结果11对夫妇取卵日平均获卵数12.3±7.1(3～24)个,成熟卵子数10.7±7.0(1～17)个,其中5例行附睾精子显微注射、6例行睾丸精子显微注射,正常受精(双原核)率为73.7%(87/118),优胚率46.2%(54/117),人均冷冻胚胎数为5.2±4.1(0～12)个,新鲜胚胎移植周期获两例妊娠(2/10),冷冻胚胎冻融移植周期(FET)妊娠2例,目前已经活产1男3女的健康小孩,尚有5对夫妇有胚胎冻存。结论11例取卵日因各种原因取精困难,经过多种途径获取精液均失败的患者,5例从附睾和6例从睾丸获取精子,从而完成受精过程。
Objective To analyze and summarize the laboratory and clinical outcomes of patients underwent intracytoplasmic sperm injection-embryo transfer with difficulty to procure semen on the day of oocyte pick-up（OPU）.Methods Assisted reproduction outcomes in 11 couples performed with the intracytoplasmic sperm injection（ICSI）with the sperm from the percutaneous epididymal sperm aspiration（PESA）or testicular fine needle aspiration（TFNA） were collected after the failure of semen retrieval on the day of OPU.Results The average number of retrieved oocytes and mature number were（12.3±7.1）（3-24）,（10.7 ± 7.0）（1-17）,respectively on the day of OPU of 11 couples.Five cases had been fertilized with the sperm from the epididymis and six cases from testicle.Normal fertilization（2 pronuclear） rate and top quality embryo rate were 73.7%（87/118）and 46.2%（54/117）,respectively.The per number of frozen embryos were（5.2 ± 4.1）（0-12） among them.Up to now,two cases of pregnancy were made from fresh embryo transfer cycles and two from frozen embryo transfer cycles,and had been delivered healthy children of one boy and 3 girls.There were also five pairs of couples with frozen embryos stored to thaw.Conclusion Of 11 patients who failed to procure semen through a variety of ways for various reasons on the day of OPU,5 cases procure sperm from epididymis and 6 from testicle for ICSI to complete the fertilization process.
