18F-FDG符合线路正电子显像对非小细胞肺癌分期的影响

摘要 目的探讨18F-FDG符合线路正电子显像(hPET/CT)对非小细胞肺癌(NSCLC)临床分期及治疗策略的影响。方法经病理组织学证实的56例非小细胞肺癌初诊患者,所有患者均行hPET/CT,和CT图像对比分析。结果56例肺癌患者,肺癌原发病灶hPET/CT检出灵敏度100%,表现为结节状或团块状病灶,病灶直径0.8~7.8cm,hPET/CT所见高代谢病灶与CT所见占位性病变位置及形态基本匹配。在转移灶的检出方面,hPET/CT在小淋巴结转移灶、骨转移检出优于CT。hPET/CT显像使非小细胞肺癌分期升级18例,其中检查后发现对侧纵隔有转移8例,发生肝转移1例,发生骨转移3例;分期降级1例。hPET/CT改变了58.6%(19/56)NSCLC患者的临床分期,使27.6%(7/29)患者的治疗策略由根治性治疗改为姑息性治疗。结论18F-FDG符合线路正电子显像使非小细胞肺癌临床分期更精确,治疗更合理。
