摘要
目的评估符合线路正电子显像在肺内孤立性结节中的诊断价值。方法对63例肺内结节行18F-FDG符合线路正电子显像检查,阳性显像者手术切除,阴性显像临床可疑者,行手术或肺内穿刺。结合术后或肺内穿刺病理,判定符合线路正电子显像检查的灵敏性,特异性和正确性。结果63例患者中阳性显像36例,其中病理证实阳性者31例,假阳性5例。阴性显像27例,其中病理证实真阴性24例,假阴性3例。灵敏性为91.2%(31/34),特异性为88.9%(24/27),正确性为87.3%(55/63),真阳性率为86.1%(31/36),假阳性率13.9%(5/36),真阴性率88.9%(24/27),假阴性率11.1%(3/27)。结论符合线路正电子显像作为一种安全、非创伤性的显像方法在鉴别肺内孤立性结节(SPN)性质时具有高敏感性,高特异性,极大地减少了不必要的诊断性开胸手术,因而有十分广阔的应用前景。
Objective: To evaluate the diagnostic value of positron emission tomograhy (PET) for patients with solitary pulmonary nodules. Methods:Total of 63 patients with solitary pulmonory nodules were examined by 18F- FDG - PET. The positive nodules identified by PET were treated by surgical removal ; The negative nodules with clinical suspected lung cancer were performed surgery or a pulmonary puncture. Through the results of surgery or puncture pathology, to determine the sensitivity, specificity and accuracy of 18F - FDG - PET imaging. Results: Among 63 cases, 36 cases were positive, including 31 cases of pathological confirmed positive, false -positive five cases ,27 cases were negative , of which 24 cases were really negative by the pathological confirmation, fase negative three cases. The sensitivity,specificity and accuracy of 18F - FDG - PET imaging were 91.2% ( 31/34 ), 88.9% ( 24/27 ), 87.3% (55/63). Conclusion :18F -FDG -PET imaging is a safe and non - injuried method in the identification of SPN, with high sensitivity and high specificity, greatly reducing unnecessary chest surgery for diagnosis.
