摘要
目的探讨膝关节镜下治疗胫骨平台骨折SchatzkerⅠ～Ⅲ型骨折的临床效果。方法将17例胫骨平台SchatzkerⅠ～Ⅲ型骨折患者采用关节镜监视下对塌陷移位的骨折进行复位,恢复关节面的平整,支撑和填塞植骨,空心钛钉固定,同时处理合并伤。结果所有患者术后均无创口或关节感染、皮肤坏死、创口不愈合或骨筋膜室综合征出现,骨折均获解剖复位或近解剖复位,优14例,良3例。结论膝关节镜是辅助治疗胫骨平台骨折的有力工具,具有损伤小、观察关节面准确、可同时治疗关节内的其他病变等优点。
Objective To evaluate the clinical therapeutic effects in the treatment of Schatzker Ⅰ-Ⅲ tibial plateau fractures under arthroscope. Methods 17 patients with Schatzker Ⅰ-Ⅲ tibial plateau fractures got reduction and smooth articular facet under arthroscopic surveillance by means of sustaining and tamponage to get bone grafting and by using hollow titanium screw to get fixation. Results All the patients were healed within 3 -4 months,with no severe complications such as poor wound healing or infection of osteofascial compartment syndromes. According to the Rasmussen criteria, the functions of the knee joints were rated as excellent in 14 patients and good in 3 patients. Conclusion The treatment of tibial plateau fractures under arthroscope can produce slight damage and is safe in operation, and joint complications can be treated at the same time.
