甘露醇联合黄角颗粒治疗脑出血并脑水肿的疗效观察 被引量:2

Efficacy of Mannitol Combined with Huangjiao Grains in Treatment of Spontaneous Intracerebral Hemorrhage

摘要 目的:探讨中西药结合治疗自发性脑出血的疗效。方法:59例自发性脑出血患者随机分为中西药组30例和西药组29例,前者采用甘露醇和黄角颗粒联合治疗,后者仅采用甘露醇治疗,余一般常规治疗处理均相同。采用BORN-BE脑水肿无创动态监护仪在发病后第1、3、5、8天分别测定2组患者患健侧大脑半球的综合扰动系数,并进行对比、分析。结果:与同组健侧比较,发病第1天患侧综合扰动系数较低(P<0.01),第3、5天明显增高(P<0.01),第8天略高。与西药组患侧比较,中西药组患侧综合扰动系数在发病第1天略高,但在第3、5、8天明显降低(P<0.05)。结论:中西药结合治疗明显优于单纯西医治疗,可显著减轻脑出血后脑水肿形成。BORN-BE脑水肿无创动态监护仪可在一定程度上评估脑出血后脑水肿的演变过程,对临床诊断及治疗有一定的指导意义。 Objective:To study the efficacy of mannitol combined with HuangJiao grains in treatment of spontaneous intracerebral hemorrhage.Methods: 59 patients with spontaneous ICH were divided into two groups.The experimental group of 30 patients were given Huang Jiao grains and mannitol solution,while the control group of 29 patients were only given mannitol therapy.The changes of comprehensive agitating coefficient of all the patients were detected by non-invasive dynamical cerebral edema monitor at days 1,3,5 and 8 after ICH.Results: Compared with the comprehensive agitating coefficients in the unaffected side of the same group,those in the hemorragic side were much lower at the time of admission(P〈0.01),but became much higher at days 3 and 5(P〈0.01).followed by returing to normal at day 8(P〉0.05).However,the comprehensive agitating coefficients in the focus side in the experiment group were slightly higher at the time of admission(P〉0.05),but much lower at all the studied time points(P〉0.05) than those in the controls.Conclusion: Combined treatment of mannitol and Huangjiao grains shows better outcome in ICH patients than the single treat-ment.Non-invasive dynamical cerebral edema monitor can evaluate the formation of cerebral edema after ICH to some extent and the effectiveness of different treatments,and is useful in monitoring spontaneous ICH.
