连续压力流率测定在前列腺增生诊断中的应用

摘要 目的探讨前列腺增生症(BPH)伴膀胱出口梗阻(BOO)患者的逼尿肌收缩状况。方法采用连续两次压力-流率法检测71例BPH伴BOO者。将在第1次压力-流率测定中得A-G值,按照Schafer氏BOO分级把病人分成3组:第1组为BOO2级19例;第2组为BOO3-4级24例;第3组为BOO5~6级28例。再将在连续两次压力—流率测定中主要参数按组别进行比较。结果第1组中第2次的膀胱最大容量和残余尿量显著增加,最大尿流率(Qmax)和最大尿流率时的逼尿肌压(PdetQmax)均比第1次显著下降;第2组中第2次的Qmax和PdetQmax比第1次显著下降;第3组中仅第2次的PdetQmax比第1次显著下降。结论本方法能准确反映每个患者逼尿肌的实际状况,为治疗方法的选择提供可靠依据。 Objective To study the condition of detrusor contractility of the patients who suffered from benign prostatic hyperplasia (BPH) with bladder outlet obstruction (BOO).Methods According to Schafer’s BOO grade and A G values measured during the first pressure flow test from two sequential studies in 71 cases of BPH, all cases were divided into three groups: Group 1 (BOO Ⅱ grade),19; Group 2 (BOO Ⅲ Ⅳ grade), 24; and Group 2 (BOO Ⅴ Ⅵ grade),28 cases.The major urodynamic parameters obtained in two sequential pressure flow tests were compared with each group.Results The second time maximum cystometric capacity and residual urinary volume were significantly increased in Group 1, while maximum flow rate (Qmax) and the detrusor pressure at maximum flow rate (Pdet Qmax) were significantly decreased than that in the first one. The 2nd Qmax and Pdet Qmax in Group 2 were evidently decreased than that in the first one. Only the 2nd Pdet Qmax in Group 3 was evidently decreased than that in the first one.Conclusion The method might represent the actual condition of detrusor in every patient and it might give a reliable basis of the option of treatment and the evaluation of its therapeutical effect.
