锁定加压钢板治疗肱骨干骨折

Treatment of minimally invasive locking compression plate to humeral shaft fracture

摘要 目的:探讨肱骨锁定加压钢板(1ocking compression plate,LCP)微创治疗肱骨干骨折的效果。方法:2005年1月~2008年1月于肱骨前方入路下行LCP微创治疗16例肱骨干骨折。男11例,女5例;年龄32~66岁。其中4例摔伤,12例车祸伤。根据AO分型:A型2例,B型l0例,C型4例。桡神经损伤2例。骨折位于肱骨中上段11例,肱骨中下段5例。伤后48h~4d手术,均无开放性骨折。结果:术后均随访6~12个月,平均8.7个月。X线片检查示,骨折均于12~l6周愈合,平均l3.6周。2例桡神经功能术后l2~36周完全恢复,平均14.6周。肩关节功能按Neer评分优9例,良4例,中3例;根据Mayo肘关节评分标准,患者肘关节功能均为优。结论:肱骨前方入路下结合LCP的微创术是一种治疗肱骨干骨折安全有效的方法。 Objective: To investigate an effect of the minimally invasive locking compression plate (LCP) treat the humeral shaft fracture.Methods: 11 males,5 females;32-66 years old, with a humeral shaft fracture were treated with the minimally invasive LCP from Jan 2005 to Jan 2008.12 of the patients were injured in a traffic accident and the remaining 4 were wounded in a fall. According to AO classification,2 fractures were type A, 10 type B, and 4 type C. The radial nerve injury was found in 2 patients. As for the fracture site ,the middle-and (or)the upper-part fracture of the humeral shaft was found in 11 patients, and the middle and (or) lower-part fracture of the humeral shaft was found in 5 patients.The patients underwent the operation in the period from 48 hours to 4 days after the injuries.Results: The patients were followed up 6- 12 months with an average of 8.7 months.The X-ray films showed that the union of fractures was achieved 12-16 weeks, 13.6 weeks on average.The function of the radial nerves recovered completely 12-36 weeks,14.6 weeks on average in 2 patient.According to the Neer Scoring System for the shoulder function evaluation, 9 patients had an excellent result, 4 had a good result and 3 had a fair result. According to the Mayo elbow performance score system 16 patients had an excellent result. Conclusion:The minimally invasive locking compression plate under the anterior humeral approach to treat the humeral shaft fracture is an effective and safe method of the humeral shaft fracture.
