摘要
目的探讨应用半导体激光微创治疗前连合受侵声门癌的适应证及临床疗效。方法对1998~2003年期间我科选择性的应用半导体激光治疗的32例前连合受侵声门癌病例进行临床分析。结果本组声门型喉癌的5年生存率为97%,复发率为16%,喉功能保留率为100%。5例复发,复发时间均在1年内,4例侵犯前连合声门下的T2病变复发率为100%,行喉次全切除环舌会厌固定术保留喉全部功能。结论有选择的应用半导体激光微创治疗前连合受侵声门癌具有创伤小、治疗时间短、费用低、嗓音恢复好等特点,虽然局部控制率有所降低,但只要随访到位,及时进行挽救手术,仍能获得满意的疗效。而对于侵犯前连合声门下的T2以上病例,目前仍应作为禁忌证。
OBJECTIVE To investigate the application and clinical effect of the minimally invasive treatment of glottic cancer involving the anterior commissure with semiconductor laser. METHODS The clinical data of 32 cases with glottic cancer involving the anterior commissure treated with semiconductor laser in our hospital from 1998 to 2003 were studied retrospectively. RESULTS No patients were lost to follow-up. The 5 year survival rate was 97%. There were 5 cases recurred. The recurrence rate was 16%. Four of them were T2 stage of glottic cancer involving the subglottic area of the anterior commissure. But all the recurrent patients preserved the laryngeal function after salvage supracricoid laryngectomy with cricohyopexy or cricohyoepiglottopexy. CONCLUSION Selectively using semiconductor laser for glottic cancer involving the anterior commissure is less invasive and cost, and can recover quickly and preserve better voice. The contraindications are T2 stage of glottic cancer involving the subglottic areas of anterior commissure.
