急性药物中毒患者血清肿瘤坏死因子α和白细胞介素8的变化及临床意义 被引量:5

摘要 目的探讨急性药物中毒患者血清肿瘤坏死因子α(TNF-α)及白细胞介素8(IL-8)水平的变化及其临床意义。方法采用放射免疫分析法测定42例急性药物中毒患者入院即刻、入院后24小时和72小时血清TNF-α和IL-8含量,42例患者按病情轻重分为一般组(24例)和危重组(18例),按中毒种类分为安眠药组(18例)和农药组(24例)。结果危重组患者各时段血清TNF-α和IL-8含量均显著高于一般组(均P<0.01);不同药物所致中毒患者之间血清TNF-αI、L-8含量变化差异无统计学意义(均P>0.05);各组患者血清TNF-αI、L-8含量均随中毒时间延长而逐渐增高。死亡组患者血清TNF-αI、L-8含量明显高于存活组,TNF-α(2.062±0.183)μg/L vs(0.718±0.314)μg/L,IL-8(0.924±0.117)μg/L vs(0.716±0.083)μg/L(均P<0.01)。结论急性药物中毒患者在应激状态下细胞因子大量释放,血清TNF-αI、L-8含量随中毒时间延长逐渐升高,升高幅度与毒物种类无关,与预后呈负相关。血清TNF-αI、L-8水平可作为判断急性药物中毒患者病情及预后的血清学指标。 Objective To investigate the change and clinical significance of serum tumor necrosis factor-α(TNF-α) and interleukin-8(IL-8) in acute drug poisoning patients. Methods The concentrations of serum TNF-α and IL-8 of 42 patients who suffered from acute drug poisoning were assayed by radioimmunoassay instantly and at the 24th and 72th hour after admission and were compared between and within different groups. The 42 patients in this research were divided into an ordinary poisoned group( n =24) and a serious poisoned group( n = 18) according to the severity of poisoning and were divided into a hypnagogue group( n = 18) and a pesticide group( n = 35) according to the sorts of poison they ingested. Results The concentrations of serum TNF-α and IL-8 in serious poisoned group at each time point were significantly higher than those of the ordinary poisoned group (all P 〈 0.01). There were no statistic differences of the items mentioned above among different drug poisoned patients (all P 〉0.05). Compared within different groups,the concentrations of TNF-α and IL-8 in the serum from the patients who suffered from acute drug poisoning instantly and at the 24th and 72th hour after admission had a tendency to increase gradually with the pathological process (all P 〈0.01). But the concentrations of serum TNF-αand IL-8 in death group were obviously higher than those of the survival group,TNF α(2. 062±0. 183) μg/L vs (0. 718±0. 314)μg/L,IL-8(0. 924±0. 117) μg/L vs (0. 716±0. 083) μg/L(hoth P 〈0.01). Conclusion The concentrations of serum TNF-α and IL-8 of acute drug poisoning patients increase at the status of stress and the indexes can serve as indicators to predict the prognoses of the patients.
