摘要
目的为骨质疏松患者寻找一种快速缓解腰背抽搐样疼痛的治疗方法。方法回顾分析2006年8月至2009年5月我科收治的出现腰背抽搐样疼痛的骨质疏松症患者112例,均为女性,年龄61～85(72.9±10.2)岁,合并椎体新鲜压缩性骨折105例。按照治疗方法将病人分为3组:A组:29例,行抗骨质疏松的基础治疗,包括口服乐力钙、阿法骨化醇片、仙灵骨葆胶囊与曲马多胶囊,肌注鲑鱼降钙素,局部电疗;B组:50例,在A组的基础上,将口服钙换成静脉钙,加味芍药甘草汤口服;C组:33例,在B组治疗的基础上加局部小针刀治疗。比较3组在治疗7d后抽痛的缓解情况与卧床翻身功能,并比较3组患者抽痛完全缓解所需要的时间。结果治疗7d后,3组的抽痛与卧床翻身功能与治疗前相比,均有统计学意义(均P<0.01),C组与B组的抽痛缓解程度与卧床翻身功能的恢复程度比A组更明显(C组:P<0.01,P>0.05,P<0.01,P<0.01;B组:P<0.01,P>0.05,P<0.01,P<0.01),C组与B组比较有统计学意义(P<0.01,P>0.05,P<0.01,P<0.05)。3组抽痛缓解时间上,C组优于B组(P<0.01),B组优于A组(P<0.01)。结论中西医结合的内外综合治疗可以提高骨质疏松患者腰背抽痛的疗效,是缓解腰背抽痛的有效方法。
Objective To explore a quick and effective method for the treatment of lumbar or back convulsive pain of the patients with osteoporosis. Methods 112 osteoporosis patients with lumbar or back convulsive pain from August 2006 to May 2009 were analyzed retrospectively. All the patients were female and their average age （72.9 ± 10.2） years old. According to the treatment method, the patients were divided into group A （treated with Osteoformon calcium capsule, Afacalcidol tablet, XIANLINGUBAO eapsule, tramadol capsule, Calcitonin and medium frequency electrotherapy, group B （treated with the treatment above except changing oral Calcium into Calcium injection and additional Paeonia and Glycyrrhiza） and group C （treated with the treatment of group B and small needle-knife therapy）. After 7 days＇ treatment, the scores of lumbar or back convulsive pain and freeing oneself function lying in bed and the time for healing lumbar or back convulsive pain were observed. Results After 7 days＇ treatment, the scores of lumbar or back convulsive pain and freeing oneself function lying in bed of the patients in three groups reduced significantly to the ones treated before（ P 〈 0.01 ）. The scores of lumbar or back convulsive pain and freeing oneself function lying in bed of the patient in group B and group C were lower than that in groupA（P〈0.01,P〉0.05,P〈0.01,P〈0.01;P〈0.01,P〉0.05,P〈0.01,P〈0.01）, so was that group C to group B（ P 〈 0.01, P 〉 0.05, P 〈 0.01, P 〈 0.05）. To the time needing for healing lumbar or back convulsive pain, the patients in group C was less than that in group B（P 〈 0.01）, so was that group B to group C（ P 〈 0.01 ）. Conclusion Exterior and interior combined therapy containing Chinese-Western Therapy is an effective method to relieve lumbar or back convulsive pain for the Patients with Osteoporosis.
