摘要
目的总结经手术治疗主动脉瘤患者的临床监护经验。方法选择2005年1月-2008年12月行手术治疗的主动脉瘤患者30例,术后监护神经系统并发症;防治出血、渗血;严密监护心率的变化;肾功能的监测;下肢循环的观察;感染的预防。结果全组患者术后出现并发症4例,手术后并发症发生率13.3%,本组患者出现以下并发症:呼吸功能不全1例,肾功能不全1例,短暂精神症状2例。本组患者手术后1.6～6h内完全清醒,未发现肢体功能障碍及硬化斑块的脱落引起的栓塞。结论外科手术治疗腹主动脉瘤,临床效果良好,加强预见性监护能减少并发症,提高手术成功率,改善患者生活质量。
Objective To conclude the clinical experience of intensive care about 30 operations of ascending aorta and aortic arch aneurysm.Methods Between January of 2005 to December of 2008,30 cases undergoing surgery were analyzed.The main points that intensive care should focus on aftersurgery：intercurrent diseases on nervous system,Prevent and cure hemorrhage and capillary hemorrhage;Give intensive care to the change of heart rate;monitor the function of kidney;observe the circulation of lower limbs;the prevention of infection.Results In all patients,there were four with complications,The operative morbidity was 13.3%,This group of patients has suffered such intercurrent diseases：1 case of insufficiency of breathing.1 case of incapacity of kidney and 1 case of temporary psychical problems.The group of patients regained full consciousness within 1.6～6 hours after operations.And they didn＇t get such intercurrent diseases as any dysfunction of limbs and embolism caused by the falling off of arteriosclerosis plaque.Conclusion To cure ascending aorta and aortic arch aneurysm by surgery will produce good clinical effect.Strengthening foreseeable intensive care can reduce the happening of intercurrent diseases,increase the chance of successful operations and improve the life quality of patients.
