摘要
目的通过动物实验评价加替沙星治疗金黄色葡萄球菌角膜溃疡的疗效及刺激性。方法建立金黄色葡萄球菌角膜溃疡的动物模型,以生理盐水为阴性对照,0.3%氧氟沙星为阳性对照,0.3%加替沙星连续滴眼7 d,用药后6 h开始每日裂隙灯显微镜下观察、记录角膜溃疡面积并评分,同时进行病理检查。结果生理盐水组和0.3%加替沙星组之间、0.3%氧氟沙星组和0.3%加替沙星组之间角膜溃疡面积比较差异有统计学意义(P<0.05)。0.3%加替沙星组与生理盐水组眼刺激分值比较差异无统计学意义(P>0.05)。结论0.3%加替沙星对兔眼金黄色葡萄球菌性角膜溃疡有较好的疗效,优于0.3%氧氟沙星,且点眼无刺激性。
Objective Staphylococcus aureus can lead to severe corneal ulcer,and gatifloxacin was used to the treatment of infectious system diseases and keratitis. This study aimed to evaluate therapeutic effect of gatifloxacin on staphylococcus aureus rabbit corneal ulcer and its ocular irritation. Methods Corneal ulcer model was created in 30 eyes of 15 rabbits by injecting the O. 1 mL of staphylococcus aureus fluid (3 × 10^8/mL) into corneal stroma to form the milky white bubble with the diameter of 4.0 -6.0 mm. O. 3% gatifloxaein( experimental group) ,0.3% ofloxaein ( positive control group) and 0. 9% saline solution was topically administered after 24 hours 6 times per day at 2-hour interval for 18 days in 10 eyes of 3 groups respectively. Corneal ulcer area was evaluated clinically under the slim lamp and scored based on the Trousdale criteria daily,and corneal inflammatory reaction was scored based on Draize criteria. The bacteria culture for Tenon ' s secretion was taken using plate streaking. Corneal specimen were collected for histopathologieal examination. Results No statistically significant difference was found in cornel ulcer area among three groups in the first day ( F = 0.80,P = 0. 46). Corneal ulcer area was significantly increased from 3 clays through 18 clays after injection of staphylococcus aureus fluid(P 〈 0.01 ). In 18 days after injection of staphylococcus aureus fluid,corneal ulcer cured, and granulation tissue, neovaseularization and infiltration of lymphocytes were seen in gatifloxacin group;while the structure of whole layers of cornea was indistinct, and more granulation tissue, neovascularization and inflammatory cells were found in of/oxacin group. In saliue solution group, corneal ulcer was still existed, and diffuse corneal inflammation,severe inflammation response were seen under the light microscope. Statistical comparations of ocular irritation scores among thos were insignificantly different(P 〉 0.05). Conclusion 0. 3% gatifloxacin has a better therapeutic effect on staphylococcus aureus-induced corneal ulcer in rabbit than 0.3% ofloxacin. Both 0. 3% gatifloxaein and 0. 3% ofloxaein have less irritation to ocular tissue.
