多层螺旋CT冠状动脉成像图像质量的影响因素 被引量:1

INFLUENCE FACTORS ON THE IMAGE QUALITY OF MULTISLICE SPIRAL COMPUTED TOMOGRAPHY ANGIOGRAPHY IN CORONARY ARTERY

摘要 目的:探讨影响多层螺旋CT(MSCT)冠状动脉成像图像质量的有关因素。方法:对300例临床疑似冠心病病人进行平扫及增强扫描。总结不同心率及其变化和呼吸运动对冠脉图像质量影响,并观察不同心率组的最佳时相分布规律。结果:增强扫描时平均心率为64次/min。共评价冠状动脉3032节段,七组中(≤55次/min、56~60次/min、61~65次/min、66~70次/min、71~75次/min、76~80次/min和≥81次/min)不能评价(血管运动伪影、血管纤细)冠脉节段分别占冠脉总数百分比分别为:13.4%(70/522)、13.0%(91/702)、19.5%(122/626)、22.7%(100/440)、31.2%(82/263)、28.8%(74/257)和37.8%(84/222)。平均心率、节律变化大、呼吸运动与运动伪影冠脉节段数经χ^2检验图像质量有显著性差异(P=0.000)。结论:在临床应用中,心率快、心律不齐及呼吸运动是影响图像质量的重要因素之一。 Objective:To study the influence factors on the image quality of multislice spiral computed tomography angiography in coronary artery.Methods:Plain and enhanced dual-scan computed tomography in coronary artery imaging were performed in 300 cases of patients with clinically suspected coronary heart disease.According the different heart rate,all cases were divided into seven groups(every 5bpm).The imaging quality was summarized and classified as three grades,and all coronary segments classified according to AHA standards were evaluated.Results:The average heart rate of the enhanced scan was 64bpm.3032 of coronary segments were evaluated.In the seven groups(≤55bpm,56~60bpm,61~65bpm,66-70bpm,71~75bpm,76~80bpm and≥81bpm),the proportion of segments that could not be assessed because of motion artifact and minute blood vessel(〈1.5mm)were:13.4%(70/522),13.0%(91/702),19.5%(122/626),22.7%(100/440),31.2%(82/263),28.8%(74/257)and 37.8%(84/222),respectively.There was significant correlation between the number of segments per patient not assessable because of factors(motion artifact and minute blood vessel) with heart rate,rapid rhythm of heart and respiratory movement during the χ^2 test(P=0.000).Conclusion:Rapid heart rate,arrhythmia and(or) respiratory movement were major cause of artifact of the image quality.
