摘要
目的定量确定40~90岁各年龄段海马结构、杏仁核、侧脑室颞角体积的正常值范围,为Alzheimer病(AD)早期诊断及鉴别诊断提供参考指标。方法对2例死于无明确神经、精神疾病的新鲜尸体颅脑行MRI斜冠状位扫描,对照解剖及组织切片确定海马结构、杏仁核、侧脑室颞角的解剖结构及边界。再随机选取619例40~90岁健康志愿者行颅脑MRI斜冠状位扫描,分别测定左、右侧海马结构、杏仁核及侧脑室颞角体积大小,并逐一求出两侧体积的平均大小(均侧),再分别以颅腔体积进行各自标准化处理,经统计学处理确定其正常值范围。结果MRI能准确地、可重复性地显示海马结构、杏仁核、侧脑室颞角的解剖边缘。标准化后的海马结构、杏仁核、侧脑室颞角体积大小在40~50岁与51~60岁之间均无显著性差异(P>0.05),在61~70岁、71~80岁、81~90岁之间均存在显著性差异(P<0.05),各年龄段体积男女性别之间无显著性差异(P>0.05),侧别之间亦无显著性差异(P>0.05)。标准化后的海马结构均侧体积各年龄段均值与95%正常值范围分别为40~60岁2.76cm3与2.60~2.92cm3,61~70岁2.40cm3与2.26~2.54?
Purpose To define the range of the normal values of volume of hippocampal formation (HF), amygdala(AM) and temporal horn(TH) of lateral ventricle of different age groups from 40 years to 90 years, in order to provide normal index for MRI diagnosis and differential diagnosis in the early stages of Alzheimer disease(AD). Methods The heads of two cadavers who died without neurological and psychological illness were examined on a 1.5T MR unit( Siemens, Magnet vision 3.1) using oblique coronal sections (repetition time/echo time 500/15 ms,slice 2mm). The MR images of HF, AM, TH of lateral ventricle were compared with anatomic and histologic sections. Volume measurements of the HF,AM, TH, normalized for total intracranial volume(TIV) were obtained on oblique coronal sections (repetition time/echo time 500/15 ms, slice thickness 2mm, no gap)and sagittal T 1 weighted images (repetition time/echo time 500/15 ms, section thickness 5mm, no gap) for 619 healthy volunteers. The margin of HF, AM,TH were outlined manually on the oblique coronal MRI sections for each side. The volumes of HF, AM, TH were separately added together to get the volume of the whole HF,AM,TH on each side and the average. The TIV was obtained on T 1 sagittal section. The HF, AM, TH volumes were standardized by TIV respectively. Results The standardized volumes of HF, AM, TH were significantly differentamong the age groups from 61 years to 70 years, 71 years to 80 years, 81 years to 90 years, and were not significantly different between the age groups of 40 years to 50 years and 51 years to 60 years. No significant difference were found between the two sides and the sexes for all age groups. The mean values and the ranges of 95% normal values of HF were 2.76cm 3 and from 2.60cm 3 to 2.92cm 3 for the age group 40 years to 60 years, 2.40cm 3 and from 2.26cm 3 to 2.54cm 3 for the age group 61 years to 70 years, 2.24cm 3 and from 2.10cm 3 to 2.38cm 3 for the age group 71 years to 80 years, 2.09cm 3 and from 1.93cm 3 to 2.25cm 3 for the age group 81 years to 90 years; also, the mean values and the ranges of 95% normal values of AM were 1.92cm 3 and from 1.78cm 3 to 2.06cm 3 for the age group 40 years to 60 years, 1.59cm 3 and from 1.45cm 3 to 1.73cm 3 for the age group 61 years to 70 years, 1.44cm 3 and from 1.30 cm 3 to 1.58cm 3 for the age group 71 years to 80 years, 1.30 cm 3 and from 1.16 cm 3 to 1.45cm 3 for the age group 81 years to 90 years; The mean values and the ranges of 95% normal values of TH were 0.10cm 3 and from 0.02cm 3 to 0.18cm 3 for the age group 40 years to 60 years, 0.20cm 3 and from 0.12cm 3 to 0.28cm 3 for the age group 61 years to 70 years, 0.31cm 3 and from 0.22cm 3 to 0.38cm 3 for the age group 71 years to 80 years, 0.41cm 3 and from 0.32cm 3 to 0.48cm 3 for the age group 81 years to 90 years. Conclusion There are different ranges of normal volume values corresponding to different age groups for HF, AM and TH, so the age factor should be considered in the research of AD, especially in the diagnosis of early stages of AD.
