轻度Alzheimer型痴呆海马结构等的MRI定量与临床智能精神检查相关性研究 被引量:10

A Correlative study of quantitative MRI of hippocampal formation with clinical mental examinations in mild dementia of Alzheimer type

摘要 目的探讨轻度Alzheimer型痴呆(dementiaofAlzheimertype,DAT)海马结构、杏仁核、侧脑室颞角MRI体积、横向弛豫时间与临床智能精神检查的相关性。方法使用Siemens1.5T超导MRI扫描机,对23例临床诊断轻度DAT的左右侧海马结构、杏仁核、侧脑室颞角体积及海马结构、杏仁核横向弛豫时间进行定量测量,然后求出每例DAT上述结构标准化后的平均体积及横向弛豫时间,对照其相应的三种临床智能精神检查:简易精神状态检查(minimentalstateexamination,MMSE)、痴呆分级量表(dementiaratingscale,DRS)、信息记忆集中试验(informationmemoryconcentration,IMC)得分,进行统计学相关分析处理。结果海马结构、杏仁核体积与MMSE均高度相关,与DRS中度相关,与IMC中度负相关,侧脑室颞角体积及海马结构、杏仁核横向弛豫时间与MMSE、DRS、IMC都无显著相关性;海马结构、杏仁核体积与相应海马结构、杏仁核横向弛豫时间之间都无显著相关性;MMSE与DRS中度相关,与IMC中度负相关,DRS与IMC无显著相关性。? Objective To study the correlations of quantitative MRI of hippocampal formation(HF), amygadala (AM) with clinical mental examinations in mild dementia of Alzheimer type. Methods Volume measurements of the HF, AM, temporal horn of lateral ventricle (TH), normalized by total intracranial volume were obtained on oblique coronal sections (1.5 T, 500/15[repetition time/echo time], 2 mm) and measurements of the transverse relaxation time of the HF, AM were obtained on oblique coronal sections (1.5T, 3 000/90/160[repetition time/echo time, echo time], 5 mm) of 23 cases of mild dementia of Alzheimer type (DAT). The patients were examinated separately by using the three clinical mental examinations of mini mental state examination (MMSE), dementia rating scale (DRS) and information memory concentration (IMC), then correlates between quantitative MRI of HF, amydala, TH and clinical mental examinations was performed by SPSS software. Results Significant correlates were observed between the volumes of HF, AM and MMSE, DRS, IMC respectively (high correlates between the volumes of HF, AM and MMSE respectively, moderate correlates between the volumes of HF, AM and DRS, IMC respectively). No significant correlates were found between the volume of TH and MMSE, DRS, IMC. Transverse relaxation time of HF, or AM did not correlate with MMSE, DRS and IMC respectively. The volumes of HF, AM were not significantly correlated with transverse relaxation time of the HF, AM. The MMSE showed significant correlation with DRS and inversely significant correlation with IMC, however, no correlation of DRS with IMC was showed. Conclusions MMSE was the first seleefive method of choice of clinical mental examinations to screen for the mild DAT, and the volumes of HF, AM were the two MRI quantitative indices that were closely correlated with clinical mental examinations in the mild DAT.
