经皮二尖瓣球囊扩张术后心电图改变与血液动力学相关研究 被引量:1

Effect of pulmonary dynamic changes on QT interval and P wave after percutaneous transluminal mitral commissurotomy

摘要 对40例风心病二尖瓣狭窄患者(具窦性心律)行球囊导管二尖瓣扩张术,术后即刻有QT间期延长(术后0.37±0.03s,术前0.35±0.02s,P<0.0001),P波振幅降低和时间缩短;肺动脉平均压减低(3.42±0.84kPa较4.82±1.48kPa,P<0.0001),肺循环血管阻力下降(130.93±90.14dynscm-5较170.52±152.14dynscm-5,P<0.05)和左心室舒张末期压增加(1.35±0.56kPa较1.19±0.55kPa,P<0.05)。线性相关和多元回归分析显示QT间期改变与肺循环阻力减低显著相关(r=-0.53,P<0.001)。P波的改变可能与肺动脉平均压下降有关。本研究指出肺循环血液动力学改变可能是二尖瓣扩张术后心电图改变的主要原因。 Successful pertcutaneous balloon valuloplasty for 40 patients with mitral stenosis and sinus rhythm resulted in QT prolongation (0 37±0 03s versus 0 35±0 02s, P<0 0001)and P wave change.Pulmonic mean pressure ( from 4 82±1 48kPa to 3 42±0 84kPa,P<0 0001) and pulmonary vascular resistance ( from 170.52±152.14dyn scm -5 to 130.93 ± 90.14dyn s cm -5 ,P<0 05)were decreased,but left ventricular end-diastolic pressure(from 1.19±0.55kPa to 1.35±0.56kPa,P<0 05) was increased after mitral commissurotomy.linear regression analysis suggested a strong correlation between QT interval prolongation and decreasing pulmonary vascular resistance(r=-0.53,P<0 001).The P wave changes had certain relations with lowering of pulmonic mean pressure.It is concluded that changes of pulmonary dynamics might play an important role in the changes on eletrocardiogram after valuloplasty
