国产膜肺与进口膜肺在离体转流中对异丙酚浓度的影响

The effect of domestic membrane oxygernator and polystan membrane oxygernator on plasma concentration of propofol during CPB

摘要 目的:探讨国产膜肺与进口膜肺分别在体外循环离体转流中对异丙酚浓度的影响。方法:选择40例择期体外循环心内直视手术患者,分别组成体外循环回路进行术前密闭式离体转流试验,用反向高效液相法测定1、5、10、20、40、60 min时的异丙酚浓度。结果:异丙酚浓度在离体体外循环开始1min时明显降低。国产膜肺组降低82.8%,进口膜肺组降低85.1%,10min后降低速度减慢,在60 min内国产膜肺组共降低98.0%,进口膜肺共降低98.3%,在1、40、60 min时,2组之间比较差异有统计学意义(P均<0.05)。结论:国产膜肺与进口膜肺一样,对异丙酚有吸附作用,但在离体体外循环中国产膜肺对异丙酚浓度的影响均要小于进口膜肺。 Objective: To investigate the effect of domestic membrane oxygernator and Polystan membrane oxygernator on plasma concentration of propofol during cardiopulmonary bypass (CPB). Methods: 40 patients for elective cardiac valve replacement undergoing open heart surgery under CPB were randomly divided into the domestic membrane oxygernator group (n=20) and the Polystan membrane oxygernator group (n=20). Two extracorporeal circuits with domestic membrane oxygernator and Polystan membrane oxygernator were compared for their capacity to absorb propofol in vitro. Blood samples were taken during CPB 1, 5, 10, 20, 40 and 60 min for the measurement of propofol plasma concentration by reverse high performance liquid chromatography (HPLC). Results: The propofol concentrations were decreased dramatically 1 minuter after CPB in the two groups in vivo, with the domestic membrane oxygernator group decreasing by 82.8% and the Polystan membrane oxygernator group by 85.1%. The decreasing propofol concentrations were slow after 10 minutes. The domestic membrane oxygernator group decreased by 98.0% and the Polystan membrane oxygernator group differences between the two groups were found in 1, 40 by 98.3% in 60 minuters. Statiseally singnificant , 60 min (P〈0.05). Conclusion: Although the domestic membrane oxygernator circuit absorbed as high as the polystan membrane oxygernator, the effect of domestic membrane oxygernator on plasma concentration of propofol was less than that of polystan membrane oxygernator in vitro.
