应用“四点一线手算法”立体定向活检诊断颅内复杂占位病变 被引量:1

Stereotactic biopsy diagnosis for complex brain tumors by the calculation of "cranium point and 3D long axis in one line"

摘要 目的观察"四点一线手算法"在立体定向活检诊断颅内复杂疾病中的应用价值。方法CT引导下取得三维坐标系统中病灶图像,与MRI图像融合。应用"立体定向四点一线手算法",取得一次贯穿入颅点、病灶2侧周边端点和病灶中心(或实体部分),共4个点的三维长轴,确定立体定向手术穿刺路径。术后行病理学检查。结果本组12例患者,大脑胶质瘤病4例,脑结核瘤1例,多发胶质瘤1例,多发恶性肿瘤1例,小脑髓母细胞瘤幕上转移1例,内囊星形细胞瘤1例,丘脑星形细胞瘤1例,脑梗塞1例,转移性腺癌1例。术后未出现颅内血肿、颅内感染和死亡等并发症。结论立体定向"四点一线手算法"是安全、可靠的操作方法,可以提高立体定向活检诊断的检出率、有效降低并发症的发生。 Objective To observe the role of the calculation "cranium point and 3D Long Axis in one line"in stereotactic biopsy for diagnosis on complex brain tumors. Methods Get the 3D shape of the focus image from CT combine with MRI. Get the skull entering point, two apical points of tumor and the center point of tumor, total 4 points, accounding the calculation of"cranium point and 3D Long Axis in one line". Stereotactic biopsy sample tissue were treated with HE and Immunochemical staining methods. Results This group including 12 patients, that gliomatosis cerebri in 4 cases, cerebral tuberculoma in 1 case, multicentric gliomas in 1 case, multicentric malignancy in 1 cases, tentorium of cerebellum medulloblastoma ismetastatic in 1 case, intraeere- bral internal capsule in 1 case, thalamencephalon astrocytic glioma in 1 case, cerebral infarction in 1 case, metastatic adenocarcinoma in 1 case. In the whole 12 examples, there were not any complications. Conclusion The results suggest that the calculation of"cranium point and 3D Long Axis in one line" is a safe and reliable operational of method in stereotactic biopsy, it can heighten the diagnosis rate of stereotactic biopsy and avoid operation complications.
