5-氨基酮戊酸光动力疗法治疗皮肤癌及皮肤癌前病变疗效观察 被引量:6

摘要 目的:观察5-氨基酮戊酸(ALA)-光动力疗法(PDT)治疗部分皮肤癌及癌前病变的疗效。方法:50例患者首次接受ALA-PDT治疗,其中日光性角化病25例、鲍恩样丘疹病16例、鲍恩病4例、基底细胞癌(BCC)4例、口腔鳞状细胞癌1例。结果:经过2~6次ALA-PDT治疗,20例日光性角化病患者痊愈,8例鲍恩样丘疹病患者痊愈,鲍恩病4例中3例痊愈,BCC有3例痊愈,口腔鳞状细胞癌患者病灶面积缩小70%。结论:ALA-PDT治疗部分皮肤癌及癌前病变具有疗效确切、安全、损伤小、无明显不良反应的优势。对于皮损位于特殊部位、惧怕手术及术后瘢痕的患者采用ALA-PDT治疗是较好的选择。 Objective: To investigate the efficacy of photodynamic therapy (PDT) with topical 5-aminolevulinic acid (ALA) in the treatment of skin cancers and precancerous dermatoses. Methods: ALA-PDT was used to treat fifty patients, including twenty-five patients with actinic keratosis, sixteen patients with bowenoid papulosis, four patients with Bowens disease, four patients with basal cell carcinoma (BCC), one patient with intraoral squamous cell carcinoma (ISCC). Results: Twenty cases of actinic keratosis, eight cases of bowenoid papulosis, three cases of Bowen's disease and three cases of BCC were cured completely. The lesions of ISCC reduced 70% in size after twice to six treatments of ALA-PDT. Conclusions: Topical ALA- PDT is an effective, safe therapy with minor trauma for the treatment of skin cancers and precancerous dermatoses, without obvious adverse reactions. It is a good alternative for the patients being afraid of operation and postoperative scar, and those with lesions on special sites.
