摘要
目的探讨生长发育高峰期后期伴有[牙合]干扰的功能性安氏Ⅱ^1类错[牙合]的治疗方法。方法选择14例伴有[牙合]干扰的[牙合]功能性安氏Ⅱ^1类错[牙合]患者作为研究对象，前牙没有明显拥挤，年龄12～15岁，根据治疗前头颅侧位片上的颈椎成熟程度判断均处于生长发育高峰期后期。所有病例采用分步治疗方法，治疗顺序为：①上颌先用MBT固定矫治器初步排齐上前牙解除[牙合]干扰；②上颌放置附有小斜面导板的活动螺旋扩弓器导下颌向前的同时扩大上牙弓后部的宽度；③下颌放置MBT固定矫治器，使用Ⅱ类颌间牵引继续排齐整平上下牙列及改善磨牙关系。治疗前后拍摄X线头颅侧位片进行测量分析，统计学采用配对t检验。结果14例患者经过平均10个月的综合治疗后基本达到个别正常[牙合]的标准。上切牙内收，前牙覆盖减小，下切牙唇倾，SNB增加、ANB减小，[牙合]平面角、面下高、下颌升支长度及下颌体长度均有增加，以上项目均具有显著性差异；第一前磨牙间宽度和第一磨牙间宽度亦有增加，且有显著性差异。结论对于生长发育高峰期后期伴有[牙合]干扰的功能性安氏Ⅱ^1类错[牙合]，综合性治疗能取得满意的效果。
Objective To investigate the treatmnt of functional Angle Ⅱ^1 after pubertal peak. Methods Fourteen functional Class Ⅱ^1 cases were chosen as subjects（ aged from 12 to 15 years old） , the bone age of individual patients was assessed according to cervical vertebrace maturation stage, and all cases were the stage after pubertal peak . The treatment course： （1）MBT appliance was used in maxilla at first to dismantle the occlusal interference;（2） slowly maxillary expansion screws with small inclined bite plate were used to promote the mandibular growth and increase the maxillary widths of dental arch in posterior tooth arch;（3）MBT appliances were used in mandible, Class Ⅱ intermaxillary elastics were continued to align the upper and lower permanent dentition and corrected the molar relationship. Cephalomentric radiographs were traced and measured before and after treatment. T test was used to determine the effects. Results The standards of individual normal occlusion were achieved after an average of 10 months of treatment. Dental alterations comprised retrusion of the upper incisors and labial tipping of the lower incisors, overjet reduced, ANB reduced, SNB, OP-SN, ANS - Me, Co-Go, and Go-Pg were increased. The widths of the first premolars and first molars were expanded. All these were significant differences. Conclusion Compositive treatment was able to get excellent results to treat functional Class Ⅱ^1 after pubertal peak.
