摘要
目的在脓毒症急性肺损伤(ALI)大鼠模型中观察鞭毛蛋白在其体内可能的致炎症肺损伤作用,及其抗血清可能的对抗炎症损伤的作用。方法复制脓毒症大鼠模型(致伤组),用鞭毛蛋白抗血清对抗(治疗组),并设定对照组(除不结扎、不刺破、不切除盲肠外,其余处理同致伤组),于2、4、6、12、24、48h时,对比观察其动脉血氧分压(PaO2)、湿/干比值(W/D)、血清和肺泡灌洗液(BALF)中鞭毛蛋白和TNF-α含量的变化以及肺组织病理改变。结果与对照组比较:在12、24、48h时,致伤组PaO2显著降低,而W/D显著升高,差异有统计学意义(P<0.01);在6、12、24、48h时,致伤组鞭毛蛋白含量显著升高;在24、48h时,治疗组PaO2显著降低,差异有统计学意义(P<0.01),而W/D显著升高(P<0.01)。在12、24、48h时,致伤组TNF-α含量显著升高,差异有统计学意义(P<0.01);在24、48h时,治疗组TNF-α含量显著升高,差异有统计学意义(P<0.01)。致伤组与治疗组比较:在12、24h时,致伤组PaO2显著低于治疗组,差异有统计学意义(P<0.01),而W/D和TNF-α含量显著高于治疗组,差异有统计学意义(P<0.01)。镜下观察大鼠肺组织发现致伤组在12h后发生显著的病理变化,而治疗组在24h后才发生显著的病理变化。结论鞭毛蛋白是脓毒症时导致大鼠肺炎症损伤的重要致伤因素。
Objective To observe the possible lung inflammation injury role of flagellin in acute lung injury(ALI) of rat induced by sepsis and possible anti-inflammation role of anti- flagellin antibody serum. Methods To observe PaO2 ,Wet/Dry value,flagellin and TNF-α concentration in serum and in bronchus alveoli lavage liquid and pathological changes of lung in injury group, treatment group and control group at 2,4,6,12,24h and 48h. Results PaO2 was significantly lower,W/D was significantly higher in injury group at 12,24,48h; flagellin concentration was significantly higher in injury group at 6,12,24,48h, PaOz was significantly lower and W/D was significantly higher in anti- flagellin antibody serum group only at 24,48h than.in control group(P〈0.01); TNF-α concentration was significantly higher in injury group at 12,24 and 48h, and was significantly higher in flagellin antibody serum group only at 24h,48h than in control group(P〈0.01). Contrast between injury group and anti-flagellin antibody serum group: PaO2 was significantly lower in injury group, but W/D and TNF-α concentration were significantly higher in injury group at 12,24h (P〈0.01). There were some obvious pathological changes of lung in injury group after 12h and in anti- flagellin antibody serum group after 24h. Conclusion Flagellin is important relative factor in ALI of rat induced by sepsis.
