摘要
目的分析泌尿系统淋巴瘤的CT及MRI表现。方法12例泌尿系统淋巴瘤,1例接受MR平扫和增强扫描,其余11例接受CT平扫和动态增强扫描。结果12例均为非何杰金淋巴瘤,11例为全身疾病的一部分,1例为原发输尿管黏膜相关性淋巴瘤。10例发生于肾脏,分别表现为肾实质内结节或肿块样病灶、弥漫性肾实质浸润、腹膜后病变侵犯一侧肾周及其肾盂输尿管、环绕肾周软组织增厚或肿块;1例输尿管淋巴瘤,表现为输尿管软组织密度结节,其上方输尿管及肾盂梗阻性扩张积水;1例膀胱淋巴瘤,CT表现为弥漫膀胱壁增厚,腔明显变窄,伴腹膜后淋巴结增大融合。结论对无既往病史、泌尿系统病变为首发表现的病例,提高对淋巴瘤累及泌尿系统各种影像表现的认识是提高诊断正确率的关键。
Objective To analyze CT and MR manifestations of urinary system lymphoma. Methods A total of 12 patients with urinary lymphoma were enrolled. Eleven of them underwent CT plain scan and dynamic-enhanced scan, the rest one underwent MR plain scan and enhanced scan. Results Non-Hodgkin lymphoma （NHL） of urinary system was finally diagnosed in all 12 patients, including 11 presenting as parts of whole body lesions, and one as primary ureteral mucosa-associated lymphoid tissue （MALT） lymphoma. Kidney involvement was detected in 10 patients, while bladder involvement was found in 1 patient. CT or MR displayed solid or multiple masses, infiltrative lesions, contiguous retroperitoneal extension and perirenal plaque or mass, respectively in the involved kidneys. Ureteral MALT lymphoma manifested as soft tissue nodules located at the lower part of the ureter and the ipsolateral collecting system obstructive dilation. Diffuse bladder wall thickening and lumen narrowing on CT were detected in the case of bladder lymphoma. Conclusion In patients without known history of lymphoma or the abnormalities of urinary system being the first findings, improved knowledge of the image patterns of lymphoma is the key to increase diagnostic accuracy.
