摘要
目的探讨经直肠超声三维显像(3D-TRUS)在前列腺良、恶性结节诊断中的价值。方法对72例前列腺结节患者行CDFI及3D-TRUS检查,对声像图表现行血管半定量分型和血管指数(VI)定量分析。结果①恶性结节在部位、边界、回声类型以及对包膜和前列腺对称性的影响等方面,与良性结节差异显著(P<0.01);②经直肠三维彩色血管能量成像(TR3D-CPA)显示前列腺癌(PCa)较良性病变血管丰富、增粗、走行纡曲、紊乱。以TR3D-CPAⅣ级诊断PCa,敏感性为78.38%,特异性为81.03%;PCa的VI值明显高于良性病变(P<0.001)。结论3D-TRUS可为鉴别诊断前列腺良、恶性结节提供丰富的诊断信息。
Objective To assess the value of three-dimensional transrectal ultrasonography (3D-TRUS) in diagnosing benign and malignant prostate nodules. Methods A total of 72 patients with prostate nodules underwent CDFI and 3D-TRUS. Ultrasonographic images were analyzed half-quantitatively, and quantitative analysis of vascularity pattern and vascularity index (VI) was performed. Results (1)There were obvious differences between benign and malignant nodules in the position, border, echo and effects on the envelope and symmetry of prostate (P〈0.01) ; (2)Transrectal three-dimensional color power angiography (TR3D-CPA) displayed richer, more tortuous and disordered vascularity in prostate cancer (PCa) than in benign lesions. In diagnosis of PCa, the sensitivity and specificity of TR3D-CPA Ⅳ was 78. 38% and 81.03%, respectively. VI of PCa was significantly higher than that of benign lesions (P〈0. 001). Conclusion 3D-TRUS can provide abundant information for the differential diagnosis of benign and malignant prostate nodules.
