组织速度成像评价慢性心力衰竭患者右心室非同步化运动 被引量:2

Tissue velocity imaging assessment of right ventricular dyssynchrony in patients with chronic heart failure

摘要 目的应用组织速度成像(TVI)技术评价慢性心力衰竭(CHF)患者右心室心肌非同步化运动。方法采集36例CHF患者心尖四腔心切面TVI图像,应用PhilipsiE33QLAB软件获取室间隔、右心室侧壁基底段和中间段组织速度-时间曲线,分别计算4个节段间QRS波起点至收缩期峰值速度、舒张早期峰值速度时间的最大差值(ΔTs、ΔTe),经心率校正后(ΔTs-c、ΔTe-c)与20名健康人进行比较,并分析与右心室结构和功能参数的关系。结果CHF组ΔTs-c、ΔTe-c较对照组延长(P<0.05),并与部分代表右心室结构和功能的参数相关性良好。结论CHF患者右心室内部存在非同步化收缩及舒张运动,TVI可为评价右心室非同步化运动提供重要信息。 Objective To observe the value of tissue velocity imaging (TVI) in assessment of right ventricular dyssynchrony in patients with chronic heart failure (CHF). Methods Thirty-six patients with CHF and 20 healthy subjects were studied at apical four-chamber view with tissue velocity imaging. Time-velocity curves were derived at the interventricular septum and right ventricular lateral wall at both basal and middle levels using Philips iE33 QLAB software. The time to peak systolic (Ts) and early diastolic (Te) velocities were measured at the beginning of the QRS complex as the reference point. Right ventricular systolic and diastolic dyssynchrony (△Ts, △Te) were defined as the maximal differences in Ts and Te between the four segments, respectively. △Ts and △Te were corrected for heart rate (△Ts-c, △Te-c) and compared with healthy subjects; the relationship among some of the structural and functional parameters of the right ventricle was also analyzed. Results △Ts-c, △Te-c prolonged in patients with.CHF compared with healthy subjects and correlated with some of the structural and functional parameters of the right ventricle in patients with CHF. Conclusion CHF patients exhibit right ventricular systolic and diastolic dyssynchrony. TVI provides important information in assessment of right ventricular dyssynchrony.
