LASEK后泪膜稳定性及角膜知觉的改变 被引量:3

Changes in Tear Film Stability and Corneal Sensitivity after Laser-assisted Subepithelial Keratectomy

摘要 目的探讨准分子激光角膜上皮瓣下磨镶术(LASEK)前后泪膜稳定性和角膜中央知觉的变化。方法76例152眼高度近视行LASEK,观察术前、术后1周,13、、6个月时的干眼症状及角膜荧光素染色(FL);测量角膜知觉、泪膜破裂时间(break-up time,BUT)和泪液分泌量(SchirmerⅠtest),并对数据进行统计学分析。结果术后1周、1个月干眼发生率及角膜FL与术前相比差异有统计学意义(P<0.01);术后1周,1、3个月角膜知觉较术前明显下降(P<0.01),BUT较术前明显缩短(P<0.05或<0.01),但术后6个月均恢复至术前水平(P>0.05);术后1周、1个月泪液分泌量较术前明显减少(P<0.01),术后3、6个月与术前相比,差异无统计学意义(P>0.05)。结论LASEK术后可导致角膜知觉减退,泪液分泌量减少,泪膜稳定性下降,但随着角膜的愈合而逐渐恢复。 Objective To investigate the changes in tear film stability and corneal sensitivity after laser-assisted subepithelial keratectomy (LASEK) ..Methods LASEK was performed on 152 eyes of 76 patients with myopia. Subjective complaints of dry eye, corneal fluorescein staining(FL) were observed in eyes at 1st week, lst,3rd and 6th months postoperatively and corneal sensitivity, tear break-up time(BUT), Schirmer' s I were determined. Resuits There were significant differences in the subjective complaints of dry eye and FL stainning dots between before operation and after operation( P 〈 0.01 ). Compared to the time before operation, Corneal sensitivity were significantly reduceded and BUT values were significant decreased at 1st week, 1st and 3rd months after operation( P 〈 0. 01 ) relative to the preoperative level, but both recoverd at 6 months postoperatively P 〉 0.05 ). Schirmer' s I test resuh was lower at 1st week and 1st months after LASEK,but no significant change was found at 3rd,6th months post- operatively ( P 〉 0.05). Conclusion IASEK causes a decrease in tear secretion, tear film stability and corneal sensitivity that can recover with the healing of cornea.
