摘要
目的:探讨研究三维CT成像在颌面部创伤合并骨折的应用价值。方法:回顾2006年6月~2008年6月,我院门诊和住院颌面部外伤合并骨折患者38例,螺CT扫描及三维CT重建进行总结和分析。结果:3D显示17例前壁骨折中有10例合并颅面部塌陷,其内凹深度(3~5)mm不等;3例合并眶下缘骨折并累及眶下孔。3D对上颌骨粉碎性骨折的位置、范围、碎骨块的移位、眶下孔受累、尤其对不规则骨折线的畸形走行等空间信息显示清晰、直观,优于2D。3D图像除对于1例下颌升支小撕脱骨折显示不佳外,均能清晰显示下颌骨髁状突骨折的类型、骨折块移位的方向和距离、与颞颌关节窝的空间关系以及下颌升枝断端移位的程度等优于2D。结论:通过本组病例的螺CT扫描及三维CT重建是图像处理技术的一大飞跃,对颌面部骨折有其独特的应用价值。不仅能提高诊断率,发现横断面图像不能显现的一些骨折,而且对手术复位、整形外科、愈后判断等都有很大的帮助。
Objective:To explore and study the application value of three-dimensional CT imaging in maxillofacial trauma with fracture.Methods:Looking back 38 out-patient and hospitalization patients of maxillofacial trauma with fractures from June 2006 to June 2008 in our hospital.soural CT scanning and three-dimensional CT reconstruction summary and analysis.Results:3D showed that 10 patients with craniofacial collapse in 17 cases of anterior wall fractures, the concave depth ranging from 3 - 5mm; 3 cases merging the inferior orbital rim flacture and holes involved. For the location, scope, bone block translocation, infraorbital pore involvement, spatial information especially for arregular fracture lines running deformities of comminuted maxillary,3D showed clearly, intuitively, better than 2D.In addition to 1 case of ramus small avulsion fracture showed the poor things, 3D images was better than 2D to clearly show the mandibular condylar fracture type, the direction and distance of fracture block shift, spatial relationship of the temporomandibular joint fossa and mandibular as well as the degree of translocation of stump or branch, Conclusion:Through eases of this group,three-dimensional spiral CT scanning and CT reconstruetion are a big leap in image processing technology;with its own unique application value for maxiltofaciat fractures.Not rely improve diagnosis rates and found a number of fractures that cross-sectional images could not appear, but also have become of great help for reduction surgery, plastic surgery, the judge after cured and others.
