摘要
目的:评价氟比洛芬酯用于妇科腹腔镜术后镇痛的有效性和安全性。方法:150例妇科腹腔镜手术患者,随机分为氟比洛芬酯组、曲马多组和对照组,每组50例。氟比洛芬酯组和曲马多组患者在手术开始前分别静脉注射氟比洛芬酯50mg和曲马多50mg,对照组则缓慢静脉注射生理盐水5ml。采用视觉模拟法(VAS评分)评价患者术后2、4、8、24h疼痛评分,记录有无呼吸抑制、恶心呕吐、头晕等不良反应及术后24h使用镇痛药的次数,同时分别在术前及术后24h进行血小板计数及凝血四项检测。结果:氟比洛芬酯组和曲马多组术后各时点的VAS评分及使用镇痛药的次数均低于对照组(P<0.05)。氟比洛芬酯组恶心、呕吐、头昏等不良反应的发生率明显低于曲马多组(P<0.05)。氟比洛芬酯组PLT计数术后较术前减少,PT、APTT术后较术前延长,差异有统计学意义(P<0.05);但术后PLT计数、PT及APTT均在正常范围,术后无1例异常出血倾向。结论:术前单次静脉注射氟比洛芬酯50mg可有效缓解妇科腹腔镜术后疼痛,且无明显不良反应。
Objective To evaluate the effectivity and safety of flurbiprofen used for postoperation anagesia on patients undergoing gynaeeological laparoscope. Method Total 150 cases undergoing gynaecological laparoscope were divided into flurbiprofen group, tramadol group and the control group ,50 cases in each group. Flurbiprofen 50mg and tramado150mg were injected via vein before operation in flurbiprofen group and tramadol group,respectively,and 0. 9% sodium chloride 5ml in the control group. VAS at 2h,4h,Sh and 24h after operation and frequency of anagesia drugs during 24h after operation were recorded, and adverse reaction such as respiratory depression, nausea and vomitting, dizzy were observed. Blood platelets count and blood clotting index before and 24h after operation were detected. Results VAS and frequency of anagesia drugs at different time after operation in flurbiprofen group and tramadol group were lower than that in the control group significantly（ P 〈 0. 05 ）. Adverse reaction such as dizzy, nausea and vomitting in flurbiprofen group were lower than that in tramadol group （ P 〈 0.05 ）. PLT after operation in flurbiprofen group was lower than that before operation, but PT and APTT after operation prolonged, there was a significant difference （ P 〈 0. 05 ）. But PLT, PT and APTT were in normal range, and no one case had abnormal hemorrhagic tendency. Conclusion Flurbiprofen 50mg injected via vein before operation can relieve pain of patients receiving gynaecological laparoscope, and no significant adverse effect is found.
