双源CT对下肢动脉阻塞性疾病的诊断及其临床价值 被引量:11

摘要 目的:探讨双源CT对下肢动脉阻塞性疾病的诊断价值。方法:对38例疑为下肢动脉阻塞性疾病患者行双源下肢动脉血管成像(CTA)扫描,并对13例患者1周内同时行数字减影血管造影(DSA)。将下肢血管分成12段,每段血管的狭窄程度被分为:正常、轻度狭窄、中度狭窄、重度狭窄、闭塞5个级别,并将CTA与DSA结果进行比较。结果:CTA与DSA在显示狭窄程度方面一致性非常好(Kappa值为0.896)。以DSA为参照标准,在血管节段基础上,CTA诊断下肢动脉闭塞的敏感度、特异性、准确性分别为100.0%、99.2%、99.3%,诊断管腔中度以上狭窄的敏感度、特异性、准确性分别为100.0%、96.7%、97.9%。结论:双源CTA在评估下肢动脉阻塞性疾病方面与DSA有很好的一致性,是下肢动脉阻塞性疾病的可靠诊断方法。 Objective:To discuss the efficacy of dual-source CT angiography in the assessment of lower extremity peripheral arterial occlusive disease. Methods :38 patients with suspected lower extremity arterial occlusive disease underwent dual-source CT angiography. CTA and DSA were performed in 13 patients within 1 week. The arterial system was divided into 11 segments. Each segment was classified as normal, mild stenosis, moderate stenosis, severe stenosis and occlusion. The accuracy of dual-source CT angiography and digital subtraction angiography was compared. Results:CTA and DSA had an excellent agreement in the diagnosis of lower extremity arterial occlusive disease( Kappa value of 0. 896). On persegment basis, in comparison with DSA as reference standard, CTA' s sensitivity, specificity, and accuracy were 100.0% ,99.2% ,99.3% for the detection of occlusive lesion, 100.0%, 96.7% , 97.9% for ≥50% stenostic lesion. Conclusion:There is an excellent agreement in detecting peripheral arterial obstructive disease for Dual-CTA and DSA. DSCTA is a reliable diagnostic method for the detection of lower extremity arterial occlusive disease.
