背景信号抑制扩散加权像在肿瘤性淋巴结中的诊断价值 被引量:3

Evaluation of Diffusion Weighted Imaging with Background Body Signal Suppression in Diagnosising Tumorous Lymph Node

摘要 目的探讨背景信号抑制扩散加权像(DWIBS)在肿瘤性淋巴结中的诊断价值。资料与方法32例经手术病理证实的原发肿瘤患者,其中原发性淋巴瘤3例,鼻咽癌9例,直肠癌6例,乙状结肠癌5例,肺癌3例,乳腺癌、宫颈癌各2例,食管癌、贲门癌各1例。采用DWIBS行颈部、胸腹部扫描,所得图像经最大密度投影(MIP)重组,并反相显示。结果所有病变在DWIBS上呈明显"浓聚",病变短径均>8.0mm。2例原发淋巴瘤位于双侧颈部、纵隔内,1例原发淋巴瘤位于腹腔、腹膜后、盆腔及腹股沟。7例鼻咽癌出现颈部淋巴结转移,3例直肠癌及2例乙状结肠癌伴盆腔及腹膜后淋巴结转移。2例肺癌出现纵隔淋巴结转移。2例乳腺癌伴纵隔及锁骨上淋巴结转移。2例宫颈癌分别伴腹股沟、盆壁淋巴结转移。1例食管癌及贲门癌伴腹膜后淋巴结转移。结论DWIBS对肿瘤性淋巴结具有高度敏感性,对肿瘤分期有重要价值,有利于指导临床治疗。 Objective To investigate the value of diffusion weighted imaging with background body signal suppression (DWIBS) in diagnosising tumorous lymph nodes. Materials and Methods 32 cases were confirmed with primary tumors of lymphomas in 3 cases, nasopharyngeal carcinomas in 9 cases, rectal cancers in 6 cases, colon sigmoideum cancers in 5 cases, pulmonary carcinomas in 3 cases, breast carcinomas and uterine cervix cancers in 2 cases respectively, esophageal cancer and carcinoma of gastric cardia in 1 case respectively. DWIBS were performed in all the patients. The scanning scope included neck, chest and abdomen. The original images of DWIBS were performed using maximum intensity projection and the reconstructed images were displayed using an inverted gray scale. Results All the tumorous lymph nodes showed conspicuously low signal in DWIBS. The minor axis of lymph nodes were longer than 8.0 mm. 2 primarily lymphomas located in hibateral neck and mediastinum. 1 cases of primary lymphoma located in abdominal cavity, retropcritoneum, cavitas pelvis and fold inguen. The metastatic lymph nodes of 7 nasopharyngeal carcinomas located at the neck, The metastatic lymph nodes of 3 rectal cancers and 2 colon sigmoideum cancers located at cavitas and retropertoneum. The metastatic lymph nodes of 2 pulmonary carcinomas located at mediastinum. The metastatic lymph nodes of. 2 breast carcinomas located at mediastinum and supraelavicular fossa. The metastatic lymph nodes of 2 uterine cervix cancers located at fold inguen and pelvic wall. The metastatic lymph nodes of esophageal cancer and carcinoma of gastric cardia located at retroperitoneum. Conclusion DWIBS have high sensitivity in detecting the tumorous lymph nodes. It have important value in assessing the stage of tumors and indicating the therapy.
