摘要
目的探讨乳腺癌T1期应用MRI扩散加权成像的临床诊断价值研究。方法临床选择2012年1月~2015年12月本院收治的乳腺癌T1期患者25例为研究对象,同期入选乳腺良性肿瘤25例为研究对照,进行MRI扩散加权成像检查法,后进行MRI动态增强扫描,比较MRI动态增强扫描与MRI扩散加权像检查法的诊断准确性;比较乳腺良性肿瘤与乳腺癌T1期患者的MRI加权扩散像ADC值;比较乳腺良性肿瘤与乳腺癌T1期患者的MRI动态增强减影扫描的环形强化、毛刺征、形态不规则等特征发生情况。结果 MRI扩散加权成像检查法的诊断准确率为80.0%(20例),MRI动态增强减影扫描的诊断准确率为52.0%(13例),两组比较差异有统计学意义(P〈0.05);乳腺癌T1期患者的MRI加权扩散像ADC值显著低于乳腺癌良性肿瘤,差异有统计学意义(P〈0.05);乳腺癌T1期患者的MRI动态增强减影扫描检测出环形强化、毛刺征、形态不规则等特征的发生情况显著高于乳腺良性肿瘤,差异有统计学意义(P〈0.05)。结论乳腺癌T1期患者进行MRI扩散加权成像检查可提高检查的准确性,临床效果确切,可与MRI动态增强扫描联合应用,有助于早期诊断乳腺癌,值得临床推广。
Objective To study the clinical diagnosis value of MRI diffusion-weighted image in the diagnosis of Stage T1 breast cancer.Methods 25 patients with Stage T1 breast cancer treated from July 2014 to May 2015 in our hospital were selected as observation group.At the same time period,50 cases of benign tumor were selected as control group.The accuracy based on MRI diffusion-weighted image and dynamic MRI enhancement imaging was compared.ADC value of MRI diffusion-weighted image for two groups was compared,the incidence rate of ring enhancement,spiculation,irregular form based on dynamic MRI enhancement imaging for two groups was compared.Results The accuracy rate of MRI diffusion-weighted image(80.0%,20 cases) was higher than that of dynamic MRI enhancement imaging(52.0%,13 cases) (P〈0.05).The ADC value of MRI diffusion-weighted image for observation group was lower than that of control group (P〈0.05).Based on dynamic MRI enhancement imaging,the incidence rate of ring enhancement,spiculation,irregular form for observation group was higher than that of control group(P〈0.05).Conclusion The MRI diffusion-weighted image has an obvious effect in the diagnosis of Stage T1 breast cancer.The combined therapy of dynamic MRI enhancement imaging is good for the early diagnosis of breast cancer.It is worthy of clinical promotion.
