摘要
目的评价Ｐｏｒｓｔｍａｎｎ法、Ｒａｓｈｋｉｎｄ法及弹簧栓子法对动脉导管未闭的治疗效果。方法采用Ｐｏｒｓｔｍａｎｎ法、Ｒａｓｈｋｉｎｄ法及弹簧栓子法共治疗动脉导管未闭（ＰＤＡ）４５例，其中Ｐｏｒｓｔｍａｎｎ法５例，Ｒａｓｈｋｉｎｄ法３３例，弹簧栓子法７例。所有患者在ＰＤＡ封堵后１０分钟重复胸主动脉造影，观察有无残余分流；术后３天及３～１８个月行超声心动图检查，观察ＰＤＡ有无再通及残余分流情况。结果全组４５例，总技术成功率为９６％（４３例）。Ｐｏｒｓｔｍａｎｎ法及Ｒａｓｈｋｉｎｄ法各有１例术中出现栓塞材料脱落而急诊手术；Ｐｏｒｓｔｍａｎｎ法有２例发生股动脉血栓形成。结论经导管治疗ＰＤＡ是有效的非手术方法，Ｐｏｒｓｔｍａｎｎ法已较少采用，直径≤２．５ｍｍ的ＰＤＡ首选弹簧栓子法，３～８ｍｍ的ＰＤＡ应采用Ｒａｓｈｋｉｎｄ封堵伞治疗。
Purpose To evaluate the efficiency of transcatheter closure of patent ductus arteriosus (PDA) using Porstmann technique, Rashkind method and occluding coils. Methods Fortyfive patients were treated with transcatheter closure by Porstmann technique 5 cases, Rashkind method 33, and occluding coils 7. Aortography were repeated in all patients to detect any residual shunt 10 minutes after closure. Echocardiogram was obtained 3 days and 318 months after the procedures. Results The procedural success rate of all 45 cases was 96%. Migration of the occluder occurred in 1 case with Porstmann technique and 1 case with Rashkind method. There was no motality in this study. Conclusion Transcatheter clousure of PDA is an effective nonsurgical method for treatment of PDA. Porstmann technique is seldom used nowdays. Occluding coils should be the method of choice to close the PDA with diameter 2.5 mm; Rashkind umbrella device should be used for treatment of PDA with a diameter ranging from 3 to 8 mm; for PDA with a diameter >8 mm, surgical closure should be selected first.
