摘要
目的探讨后腹腔镜肾囊肿去顶术的疗效和并发症。方法回顾性分析32例后腹腔镜肾囊肿去顶术患者的临床资料,并进行随访。结果31例手术成功,1例转开放性手术。共切除囊肿数目为46个。手术时间60～180min,平均80min。术中出血20～150mL,平均100mL。术后住院时间3～7d,平均4.5d。手术并发症包括:腹膜损伤5例(15.6%),皮下气肿1例(3.1%),尿路感染2例(6.2%),腰背部和肩部疼痛3例(9.4%),切口疼痛2例(6.3%)。随访时间1～56个月,平均23个月。B超或CT检查未见囊肿复发。结论后腹腔镜肾囊肿去顶术创伤小、安全、有效,并发症轻微,并且易于处理。
[Objective] To explore the efficacy and complications of retroperitoneal laparoscopic decortieation of renal cysts. [Methods] A retrospective review of 32 cases with retroperitoneal laparoseopie decortication of renal cysts was performed. The clinical data and fellow-up results of these cases were analyzed. [Results] The retroperitoneal laparoseopie deeortieation of renal cysts was successfully performed in 31 cases and a conversion to open surgery was required in t case. Totally 46 renal cysts were reseeted. Mean duration of surgery was 80 minutes （60- 180 minutes） and mean operative blood loss was 100 mL （20-200 mL）. Mean length of hospital stay was 4.5 （3-7） days. Operative complications included peritoneal injury in 5 eases （15.6%）, subcutaneous emphysema in 1 case （3.1%）, urinary tract infection in 2 cases （6.2%）, back and shoulder pain in 3 cases （9.4%） and incision pain in 2 cases （6.3%）. At a mean follow-up of 23 （1-56） months, renal ultrasonography or CT scan showed an absence of radiological recurrence of the renal cyst in all the cases. [Conclusion] Retroperitoneal laparoseopic deeortieation is a safe, minimally invasive, and efficacious procedure for the treatment of renal cysts. Most complications are mild and easily managed.
