摘要
目的探讨急性生理学及慢性健康状况评分系统Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)在指导冠心病监护室(coronary care unit,CCU)护士评估重危患者的应用价值,构建与实施基于APACHEⅡ评分的分级监护程序。方法对123例CCU的急性冠脉综合征和急性心功能不全患者进行APACHEⅡ评分并预测死亡率,通过对生存组和死亡组病例资料统计分析,检测APACHEⅡ评分在CCU应用的有效性。结果生存组和死亡组APACHEⅡ分值和预测死亡率差异显著(P<0.01);随治疗时间的延长生存组APACHEⅡ分值呈下降趋势,死亡组APACHEⅡ分值呈上升趋势;APACHEⅡ评分≥21分的患者死亡率高。结论APACHEⅡ评分可用于CCU内护士量化评估患者病情严重程度,为CCU分级监护提供依据。
Objective To assess the instructive value of acute physiology and chronic health evaluation Ⅱ （APACHEⅡ） score for nurses in coronary care unit （CCU） to evaluate patients with severe heart disease, to construct and develop the hierarchical monitoring system based on APACHE Ⅱ score. Methods 123 patients enrolled in CCU with acute coronary syndrom （ACS） and acute heart failure were evaluated by APACHEⅡ score,and their mortality was predicted. The validity of APACHE Ⅱ sore in CCU monitoring was statistical investigation of the scores in survival group and death group. Results There was significant difference in APACHEⅡ score between survival group and death group （P〈 0. 01 ）. As treatment continued, the score of survival group decreased while the score of the death group increased. Patients with the score more than 21 had high death-rate. Conclusion APACHEⅡ score is an effective method for CCU nurses to evaluate disease severity,and can supply beneficial reference for CCU hierachical monitoring.
