摘要
目的观察应用多波长激光联合复方血栓通胶囊治疗糖尿病性视网膜病变(DR)的有效性和安全性。方法94例(168只眼)DR患者随机分为多波长激光联合复方血栓通胶囊组(A组)和单纯多波长激光组(B组),每组47例(84只眼),根据荧光素眼底血管造影(FFA)确定光凝方式及参数。A组光凝后口服复方血栓通胶囊12个月,每3个月复查肝肾功能;B组未服用药物。随访观察疗效及副作用。结果对眼底DR疗效:A组显效19只眼(22.6%),有效52只眼(61.9%),无效13只眼(15.5%),总有效率84.5%;B组依次为14只眼(16.7%)、40只眼(47.6%)、30只眼(35.7%)及64.3%,两组总有效率差异有统计学意义(χ2=9.03,P<0.05)。视力变化情况:A组视力提高41只眼(48.8%),不变38只眼(45.2%),下降5只眼(6.0%),总有效率94.0%;B组依次为26只眼(30.9%)、43只眼(51.2%)、15只眼(17.9%)及82.1%,两组差异有统计学意义(χ2=5.68,P<0.05)。A组未观察到药物特殊毒副作用。结论多波长激光联合复方血栓通胶囊治疗DR疗效满意,二者可起到互补作用,提高并巩固治疗效果,且安全无副作用。
Objective To observe the clinical therapeutic effect of diabetic retinopathy （DR） treated by multi- wavelength laser combined with complex Xueshuantong capsule. Methods 94 cases（ 168 eyes ） were divided randomly into two groups equalily： treated by multi-wavelength laser coagulation combined with complex Xueshuantong capsule （ Group A ） , treated by simple multi-wavelength laser coagulation （ Group B ）. All 168 eyes received laser coagulation with different mode and parameter according to the fundus fluorescein angiography （FFA）. After laser coagulation, Group A taked com- plex Xueshuantong capsule orally everyday for 12 months and recheck for liver and renal function every 3 mouths, while Group B didn＇t taked drugs. Patients were followed up for therapeutic effect and side effects. Results The therapeutic effect of DR in Group A was excellent in 19 eyes （22.6%） ,effective in 52 eyes 161.9% ） ,ineffective in 13 eyes （ 15. 5% ） ,total effective rate 84.5% ; versus in Group B, excellent in 14 eyes （16.7%） ,effective in 40 eyes （47.6%） ,inef- fective in 30 eyes （35.7%） ,total effective rate 64.3%. There was significant difference between the two groups（ χ^2 = 9. 03, P 〈 0.05 ）. The visual acuity in Group A was enhanced in 41 eyes （ 48.8% ） , remained the same in 38 eyes （ 45. 2% ） , declined in 5 eyes （6.0%） , total effective rate 94.0% ;versus in Group B , enhanced in 26 eyes（30.9% ） , remained the same in 43 eyes（51.2% ） ,declined in 15 eyes/17.9% ） ,total effective rate 82.1%. There was significant difference between the two groups （ χ^2 = 5.68, P 〈 0.05 ）. There was no special drug side effects in ＇Group A. Conclusion The combination of multi-wavelength laser and complex Xueshuantong capsule plays satisfiable therapeutic effect in treating DR, the two means are mutual complementarity which can reinforce and confirm the clinical effect, and it is rather safe.
