摘要
采用三维空间有限元计算方法,研究仰卧、侧卧两种睡姿8种不同枕高情况下颈脊柱三维空间结构体内单元与节点的应力和位移的变化规律,并从生物力学应力与应变关系的角度,探讨了用枕不当与颈椎病发病的内在联系。结果表明:仰卧、侧卧两种睡姿的高枕、低枕,都会通过头颅位置的改变而对颈脊柱产生应力作用,引起颈脊柱结构体,特别是椎间盘、钩椎关节及小关节处的位移与应变,导致颈椎间盘变性、突出,关节紊乱,颈脊柱内外平衡失调等,诱发或加重颈椎病的发生。提示:临床上根据个体的睡姿习惯,科学地指导用枕选择,对于防治颈椎病,具有重要意义。
The 3-D analytical method of finite element is used in this thesis to study the distribution laws of stress and the displacements of the untis and nodes in the 3-D structure of cervical spine under the two kinds of sleep postures(lie on the back and on the side)and eight different height of pillows.Then probe into the biomechanics mechanism through the relation of stress and strain between the wrong using of pillow and the causing of cervical spondylosis.The results show that:No matter lie on the back or lie on the side,too high or too low pillows all can cause stress on the cervical spine through the change of the head postures.And the stress can bring about the structure of cervical spine to displacement and strain,especial the little joints between vetebra、luschka joints and intervetebral discs.All these can result in the intervetebral discs denaturation and protrusion、the joints between vetebra disorder and the balance in and out of cervical spine dislocation,etc,which cause the come on or strengthen of cervical spondylosis.The research indicate:On clinical work,to instruct scientifically the choice of pillows according to personal habit of sleep posture is very important to prevent and treat the cervical spondylosis
